Contractor agrees to provide Unsheltered Resident Encampment Site Waste Removal and Cleanup
services to the California Department of Transportation (Caltrans) on an on-call, as-needed basis.
Bidder must have at time of bid submittal and for the duration of this Agreement a current and valid
Motor Carrier Permit(s) (MCP) issued from the California Department of Motor Vehicles (DMV) for its services as a Motor Carrier of Property under this Proposed Form of Agreement.
Bidder and any subcontractor(s) must be registered at time of bid submittal with the Department of
Industrial Relations (DIR), pursuant
to Labor Code section 1725.5.
Contractor shall maintain the following training, permit, and licenses for the duration of this
Agreement:
Contractor License(s): Class A General Engineering license and a HAZ - Hazardous Substance Removal Certification from California Contractor¿s State License Board (CSLB).
Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration.
Hazardous Materials Transportation License issued by California Highway Patrol (CHP).
Trauma Scene Waste Management Practitioner (from State of California Department of Public Health, Medical Waste Management Program).
Waste Tire Hauler Registration issued by Cal Recycle.
Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40
hours of training.
Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control.
Confined Space Entry Certification, as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 17. A permit and contractors program documents will be required by the start of work.
Contractor or subcontractors applying or spraying disinfectant shall possess a Pest Control
Business (PCB) license and have a Qualified Applicator License (QAL) with Category K
(health related) staff to oversee the business operations. Both of these licenses are issued
by Department of Pesticide Regulation. https://www.cdpr.ca.gov. Contractor or
subcontractors applying or spraying disinfectant shall also be registered with each individual
county agricultural commission where the disinfectant will be applied (Los Angeles and
Ventura counties). Proof of these requirements shall be provided to Caltrans Contract
Manager prior to applying or spraying disinfectant.
