Maricopa County Department of Public Health (MCDPH) is seeking one or
more Contractors (hereinafter referred to as Consultants) to support
the implementation of five (5) Community Advisory Boards (CAB) that
were developed through a grant received by the Centers for Disease
(CDC) known as the Health Disparity Grant. These five (5) CABs
represent a different region of Maricopa County: Northwest, Southwest,
Central, Northeast, and Southeast. These regions were established by
MCDPH to develop, maintain, and strengthen strong partnerships with
community leaders and stakeholders to

enhance public health support.

To further support and grow
this effort, funding will be allocated (through a separate
solicitation process) to mobilize community organizations and
municipalities, and to work collaboratively to create Regional Health
Improvement Plans (RHIP)/Community Action Plans (CAP). Each regional
CAB shall have its own individual Consultant. Consultants shall
provide administrative support, coaching, structure, and facilitation
for their designated regional CAB to walk them through conception,
development, and implementation of the RHIP/CAP.  The
regional CABs are scheduled to begin on April 1, 2022 and will run for
a minimum of one year with an anticipated end date of May 31,
2023. 
 
MCDPH has allocated a total of $500,000
to be distributed among all 5 regions in the implementation of this
project. Awards shall be made as cost-reimbursement service contracts
with an anticipated project period beginning April 1, 2022 through May
31, 2023. Respondents that may require financial assistance for
initial setup costs may request an advance payment of up to $5,000
upon final contract execution. Respondents should include which region
(Northwest, Southwest, Central, Northeast, or Southeast) they would
like to support/represent as well as the reasons why. If requesting
multiple regions, responses must indicate a separate individual
Consultant for each region requested. Refer to Section 5.7 for
additional guidance on response guidelines. 
 
Ideally, awarded Respondents shall possess the following preferred
attributes listed in no particular order:
 
Minimum of three years’
experience guiding and leading teams for performance improvement and
in facilitating groups
Demonstrated knowledge of coaching
practices, adult learning theory, strategic planning, program
planning, program development and implementation, program evaluation,
the Collective Impact model, the Action Learning Team model, Quality
Improvement processes, and group dynamics
Demonstrated
understanding of Diversity, Equity, & Inclusion (DEI)
principles
Lived experience and/or have strong community
knowledge of Maricopa County priority populations.
Demonstrated
experience working with public health programs
Demonstrated experience in
conducting SWOT analyses, positive inquiry, and implementing emergent
learning tools with groups

EVALUATION AND AWARD:
Award is made at the sole discretion of the solicitation issuing
Department.  Maricopa County reserves the right to award in full or
in part to a single vendor or multiple vendors.
