The intent of this Request for Information is conducting market research for Sources Sought to determine Services Disabled Veteran Owned Small Business (SDVOSB), and Veteran Owned Small Business (VOSB) capabilities, for providing prefabricated folding, transportable medical exam units, relative to the NAICS 332331. Size standard 750 employees.This is a sources sought notice seeking information and VOSB and SDVOSB vendors that are capable of providing for fold-out, transportable medical units that contain at least 3 exam room, and an administrative room and includes a sink and HVAC, with additional specifications. The specifications are provided below for review, in the section for description of requirement. Information is requested for determination for size standard set-aside for solicitation.INFORMATION REQUESTED and SUBMITTING: Vendors interested in this RFI may provide a response of your company capabilities statement. Submittals must be brief description, no more than five (5)
pages, that demonstrates your company has a similar product, scope and capability. Respondents are solely responsible for all expenses associated with responding to this RFI.DESCRIPTION OF REQUIREMENT SPECIFICATIONS:MOBILE FOLD-OUT MEDICAL UNITSThe products should be equivalent to the manufacturer: Elite Aluminum Corporation, product C-FORTS 4 Room Medical Shelter System, product #CG964MC)Veterans Health Administration currently has a requirement to procure expandable self-contained standalone mobile fold out shelters that will serve the Veteran and civilian populations as medical clinics. The shelters would be used to create an efficient clinic system which is a model of traditional primary care. BACKGROUND: Mobile Medical Units were deployed during the 2017 Hurricane Season, to support devastated communities with emergency health care and enhance the rebuilding efforts. The VA identified during the response there was a need for additional units to support the VHA Mission and the National Response Framework requirements. The Office of Emergency Management has researched mobile medical capabilities to ensure VA Leadership is provided with the best solution for securing additional resources that are deployable, will provide the required space and equipment, and be easily transportable into the impacted area to fulfill the mission. The VA plan is to pre-position Fold Out Medical Shelters at different Veterans Affairs Medical Center locations throughout the United States. The Veteran Affairs Medical Centers will be selected based on their strategic location and the availability of the appropriate vehicles to transport each unit. The strategic locations have been identified and each location allows for these units to be deployed in a more rapid response to an incident/event. Fold Out Medical Shelter Construction The Fold Out Medical Shelter will be constructed so that each is reinforced to be able to withstand movement over the road. This reinforcement must protect the Fold Out Medical Shelter to maintain the exterior integrity allowing it to be successful in rapid deployment. Exterior of Fold Out Medical Shelter - Fold Out Medical Shelter will consist of hard side walls, roof and floor. The walls, roof and floor should fold and unfold in a manner that allows the unit to travel within the legal dimension over the road. The Fold Out Medical Shelter once expanded will consist of three (3) exam rooms and one (1) office area. Leveling of Fold Out Medical Shelter - The Fold Out Medical Shelter must have a mechanism in place to allow it to be leveled on surfaces that consist of hard stand (blacktop, concrete) as well as grass, dirt, gravel areas. The Fold Out Medical Shelter should be constructed so that a maximum of three (3) unskilled laborers can deploy/redeploy within 45 minutes Entry Ways - The Fold Out Medical Shelter will have a door for entry into the unit that enters into the office area. The exam rooms will have a door for entry/exit from inside of the Fold Out Medical Shelter. The door into the structure and doorway into the exam rooms will be Americans with Disabilities Act (ADA) compliant. All door thresholds should be designed to allow a wheelchair to maneuver without difficulty. Windows The exam rooms will have a window that can be opened to allow air circulation as necessary. The window must have a locking mechanism in place. A screen covering the open portion of the window must be provided. A window shade will be provided for privacy. Awning - The Fold Out Medical Shelter will have a deployable awning unit outside of the front door that will provide shade and shelter during rain or other inclement weather. HVAC - The Fold Out Medical Shelter will be equipped with Heating, Ventilation and Air Conditioning (HVAC) that is attached to the unit and has the duct work installed inside of the Fold Out Medical Shelter allowing sufficient air flow to cool and heat the Fold Out Medical Shelter. The Unit must be able to maintain a constant interior temperature of 68o-70o Fahrenheit, with outdoor temperatures ranging from -25o to 120o Fahrenheit. Power - The Fold Out Medical Shelter must be wired for 120/240 volt electric, 125 amp single phase convertible load center. There must be at least a 100 amp break for permanent power connections for shore power/generator. The three exam rooms must have a minimum of one 110v dual outlet located within the room. The office area will have two (2) 110 v dual outlets located within the walls. Power Connections The Fold Out Medical Shelter will have external electrical connections that will allow the unit to be connected to shore power and generator power Generator - An appropriate size generator that can power the Fold Out Medical Shelter to the above specifications will be provided. This generator will operate on diesel fuel. The generator will be outfitted in a manner that allowed it to be maneuvered by hand or other means that do not require material handling equipment (forklift etc.) Tie Down System - The Fold Out Medical Shelter will be constructed so that it can be secured during heavy winds. The building will be designed to sustain wind speeds up to 125 mph. The tie downs straps will be provided. Outside Appearance - The outside of the Fold Out Medical Shelter will be white in color and branded with the Veterans Affairs Logo on all four sides of the structure. Interior Rooms There will be three (3) exam rooms and one (1) office area inside of the Fold Out Medical Shelter. Interior Walls - The interior walls will be made of a solid material that will allow them to be cleaned and disinfected without causing any damage to the finish. Flooring - The flooring inside of the Fold Out Medical Shelter will be of a membrane material that will allow the floors to be damp mopped without causing water damage. The floor should consist of a texture that prevents slippage under dry and wet conditions. Data Ports - The Fold Out Medical Shelter will be equipped with a minimum of one data port connection in each exam room. The office area will be equipped with a minimum of two data ports. The CAT 6 cables will be routed through the walls of the Fold Out Medical Shelter and all terminate at one central location within the office area of the Fold Out Medical Shelter. The CAT 6 cables at the terminated end will have the appropriate male connectors installed. The CAT 6 cable and each data jack will be labeled identifying the destination. All data jack connections will be tested to CAT 6A standards. Exam Room Furniture The exam rooms will be equipped with the following; Exam Table one per room Adjustable Stool for provider one per room Exam Light one per roomSink Self-contained with tanks capable of holding a minimum of five (5) gallons of portable and gray water that are stored underneath of the sink. Must have connections that will allow for filling of the potable tank and to discharge the gray water tank Stationary Office Chair one per room Office Area Furniture The office area will be equipped with the following; Modular Desk Stationary Office Chair Four (4) Trailer or Method of Movement - The Fold Out Medical Shelter will be moved on a trailer that can be towed with a 3⁄4 -1 ton vehicle with a standard ball hitch or pintle hook. This trailer must be road worthy and have all appropriate lights, brake lights and turn signals. The trailer will be equipped with electric brakes and has a 7 pin connector providing electric and control to the brakes and lights. Upload/Offload of Fold Out Medical Shelter - The Fold Out Medical Shelter will be constructed to upload and off load from a trailer using only integrated mechanisms and without utilizing any additional material handling equipment such as a forklift or crane. Storage Compartments on Trailer - The trailer will have storage compartments permanently mounted to allow storage of additional equipment as deemed necessary by Veterans Health Administration Office of Emergency Management (VHA OEM). Fire Extinguishers - Fire extinguishers mounted inside according to the applicable laws and statues will be provided for each of the Fold Out Medical Shelters.Entry Ramps - Entry ramps that conform to the ADA guidelines will be provided for each of the Fold Out Medical Shelters. These ramps should have hand rails that are easily mounted on both sides of the ramp and are continuous.TRAINING The contractor will provide the appropriate training to VHA OEM staff on the proper way to load/unload the Fold Out Medical Shelter, setting up and taking down, the operational equipment inside the Fold Out Medical Shelter. Additionally, the care and maintenance of the shelter, trailer and associated equipment. The location and time of the training will be coordinated with the PM/COR.WRITTEN DOCUMENTATION The contractor will provide a comprehensive owner s manual and laminated quick operational guide for each unit.Specifications- minimum 250 250 ft- Must have towing capability, not self-propelled.-Must be certified by the National Highway Traffic Safety Administration.-Must be configured to fold out, from a legally towable configuration- Must consist of solid walls roof and floor.-Walls must consist of insulated aluminum panels.-Must consist of fixed and folding partitions creating minimum of three exam rooms and one administrative office space when folded out.-Must include a mechanism to allow for leveling with the capability to be leveled on any surface including concrete, grass, dirt, gravel and sand.-Must be constructed in such a way that three unskilled workers can set it up and put it back into a towable configuration within maximum time period of forty-five minutes without a use of a forklift.-Must include a hydraulic trailer unloading system.-Each exam room must have insulated impact operating windows with locking mechanisms, screens, and collapsible window shades for privacy. Dimensions window must be at least 42 x 52 .-Must be certified to 2012 International Building Codes.-Must have a warranty period of at least ten years.-Interior walls must have an anti-bacteriostatic finish that can be easily cleaned without damaging shelter.-The mobile clinic will be equipped with Heating, Ventilation and Air Conditioning (HVAC) that is attached to the unit and has the duct work installed inside of the Fold Out Medical Shelter allowing sufficient air flow to cool and heat the Fold Out Medical Shelter. -The Unit must be able to maintain a constant interior temperature of 68o-70o Fahrenheit, with outdoor temperatures ranging from -25o to 120o Fahrenheit. Unit must have Integrated with Interior Ductwork to each room. - Must have 120/240v, Single Phase 125 amp load center, Shore Power/Generatorconnection points. All Electric Outlets and Data Ports must be installed within the walls of theunit. Must have (2) ea.
