Midvale Avenue.Scope of Work: Sidewalk, stamped, colored crosswalk, 2 drive aprons. Excavate, saw cut, demo & haul off spoils of excavation. Form, place, & finish 150’ of 5’ wide sidewalk 6” thick of 4000psi concrete with fiber mesh reinforcement and 2” of base stone.To transition from the existing 10’ sidewalk on the eastbound side of the road to an ADA transition with a yellow pegboard at the crosswalk.To include a 12’ driveway apron with #4 rebar for the utility companies to access the powerline easement.To include 2 sidewalk cross drains with removable tops. Form, place, & finish 50’ of 5’ sidewalk 6” thick of 4000psi

concrete with fiber mesh reinforcement and 2” of base stone.From the westbound side of the road at the crosswalk to include an ADA transition with a yellow pegboard to the driveway apron to Stringer Ridge Connector Trail. The driveway apron at Stringers Ridge Connector is approximately 880sqft. 6” thick with 4000psi concrete and #4 rebar 16” on center each wayTo be tied into the existing driveway at 410 E. Midvale Ave.To include the adjustment of 2 manhole rings and lids. The stamped crosswalk should be red in color and level with the existing road grade and a total of 945sqft. (45’x 21’) of 6” thick of 4000psi concrete with #4 rebar. The center 5’ of the crosswalk should be smooth and the remaining 40’ (20’ each direction) should be stamped.
