An effective Community Health Assessment and planning process includes community engagement— collaboration between entities with diverse and relevant expertise, and participation reflective of the community served. It is essential to reach out and engage populations most affected by a particular health issue, in order to understand the contributing conditions and identify effective strategies and activities. It is necessary to seek engagement, perspectives, and input from different community populations, in order to identify and build on existing community

assets.Mariposa County is seeking proposals for technical assistance and coaching to accomplish the completion of the Community Health Assessment (CHA) and development of the Community Health Improvement Plan (CHIP). The purpose of this Request for Proposals (RFP) is to solicit proposals from qualified individuals or agencies to provide: Coordination and guidance on the completion of the CHA and creation of the final CHIP. Work contracted under this RFP will not exceed $75,000.
