Wylie VA Ambulatory Care Center, 420 N. James Road, Columbus, OH 43219 intends to solicit and negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. The VA intends to solicit and negotiate with Karl Storz, 2151 E. Grand Avenue, El Segundo, CA 90245, and with L1 Enterprises, 618 W. Patrick Street, Frederick, MD 21701 an authorized distributor for Karl Storz Scopes. The Surgery Services at the Chalmers P. Wylie VA Ambulatory Care Center is in need of the Storz HD (High Definition) flexible video cystoscopes, working elements and sterilization trays. The HD image quality and built in LED light lets the surgeon see more contrast, tissue differentiation and depth cues. The contoured shape for smooth access and

increased patient comfort. The HD flexible cystoscopes are currently used in the urology clinic and surgical suites. Additional HD flexible cystoscopes are needed for the increased number of Veterans being seen in the clinic and surgical suites. The requested requirements must be compatible to our existing Storz HD (High Definition) flexible Video Cystoscopes and Telepack Plus Video Tower. The Government believes that only these business can satisfy the agency requirements and intends to negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. Wylie VA is in need of the Storz HD (High Definition) flexible video cystoscopes, working elements and sterilization trays. The HD flexible cystoscopes are currently used in the urology clinic and surgical suites. Additional HD flexible cystoscopes are needed for the increased number of Veterans being seen in the clinic and surgical suites. The facility is adding two new urology surgeons. With the increased number of surgeons working and seeing Veterans, there will be an increased need for more equipment to allow the Veterans care to be completed without delay. The requested requirements must be compatible to our existing Storz HD (High Definition) flexible Video Cystoscopes and Telepack Plus Video Tower. Accordingly, Karl Storz is the only firm capable of providing the supplies and services described in Section III above without the Veteran s Health Administration experiencing substantial duplication of cost that could not be expected to be recovered through competition. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: An Intent to Sole Source is being posted on Contract Opportunities, 36C25024Q0115, for 3 days allowing any one to identify their interest and capability to respond. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: This is the best value option since it allows the facility to continue using all other equipment associated with this requirement. The Contracting Officer has ensured that the offered cost is consistent with the Independent Government Cost Estimate and is considered F&R. In addition, L1 Enterprises, the authorized SDVOSB, is offering the requirement for a meager 4% more than Karl Storz. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Market Research was conducted by search of VetCert and DSBS. Searched VetCert using NAICS 334510 resulted 306 vendors. Searched DSBS using the same NAICS, resulted 4,733. The adding of keyword, Cystoscopes, resulted in 0 Vendors. The VA Rule of Two is Unattainable. As described in Section 6 above, market research, in accordance with FAR Part 10, was conducted by synopsis of the proposed acquisition, advising industry of the pending acquisition and soliciting inquiries from interested parties.
