19.1.1 Tab A: Table of Contents.
19.1.2 Tab B: Qualifications: Introductory letter, to include the name of the instructor and contact information. Documentation required as per 15.1, 15.2, 15.3, 15.4, & 15.5.
19.1.3 Tab C: Rates & Fees as per 15.6.
19.1.4 Tab D: Prior history and references: List of prior contracts with other government municipalities and a minimum of three
