SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339113 (size standard of 750 Employees). Responses to this sources sought will be used by the

Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. This RFI is to provide the Department of Veterans Affairs, VISN 15, Kansas City VA Medical Center Primary Care Service with three (3) new exam bed tables. The exam bed tables must meet provided technical requirements. 2. BACKGROUND The Kansas City VA Medical Center is in the process of upgrading the Primary Care examination area. As part of the upgrade three (3) Electronic examination tables are needed for Primary Care Service area. 3. SCOPE OF WORK The contractor shall provide three (3) exam bed tables to the Kansas City VA Medical Center in accordance with the following technical requirements. 4. TECHNICAL REQUIREMENTS Requirements (shall meet or exceed): The Exam Bed Tables shall be electronic with adjustable electronic features, for the head, backrest, leg rest, set adjustment and foot functions. Extensions on Feet and headrest would be preferred. All electronic features should have a remote with easy controls for comfort of patient positioning. All features shall be independently operational. The Exam Bed Tables shall have durable medical grade seamless upholstery. The Exam Bed Tables shall have armrests that can be rotated 180 degrees back and forth. The Exam Bed Tables shall have the capacity to hold up to 350 lbs. The Exam Bed Tables shall have the option of Stirrups for Women Health examinations. The Exam Bed Tables shall meet the following requirements for size: Upright position: Minimum Height: 46 inches Max Height: 58 inches Max Length: 67 inches Flat Chair: Minimum Height: 24 inches Maximum Height: 35 inches Maximum Length: 77 inches Width including armrests: Maximum: 33 inches The Exam Bed Tables shall meet the following requirements for angles: Headrest extension: +/- headrest extension Backrest angle: 0 degree to 85 degree Seat angle: 0 degree to 15 degree Footrest angle: 0 degree to 90 degree Adjustable height up and down: 24-58 inches The Exam Bed Tables can have small storage areas underneath the chair but should not exceed the amount of one storage drawer or area. The Exam Bed Tables shall have open space around the Base of the Chair and the chair for easy cleaning. Additional Information: Please provide capabilities and technical information of proposed Exam Bed Tables. 5. DELIVERY & LOCATION INFORMATION Delivery of items shall be shipped to: Department of Veterans Affairs, VISN 15 Belton CBOC 209 Cunningham Industrial Parkway Belton, MO 64012 This delivery will include installation The information identified above are intended to be descriptive, not restrictive, and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications, or brand name equal. If you are interested and are capable of providing the sought out supplies/services, please provide the requested information as well as the information indicated below. If you do, state how and what is altered; assembled; modified? Does your company have an FSS contract with GSA, VA NAC, NASA SEWP, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW.
