The Contractor shall provide Off-site Temporary Residential Housing Shelter for Homeless Veterans Service for the beneficiaries Salem VA Health Care System Medical Center, Salem, VA.Place of Performance: Salem VA Health Care System, 1970 Roanoke Blvd., Salem, VA 24153.Located in the community or within a 25-miles radius of the Salem VA and located within a 25-miles radius of the Community Based Outpatient Clinics (CBOC) and/or Health Care Centers (HCC). with Five (5) beds. The Department of Veterans Affairs, Salem VA Health Care System Medical Center in Salem, Virginia requires Contractors to provide Off-Site Temporary Residential Housing Shelter services in the community as part of its Community Base Health Care for Homeless Veterans (HCHV) program and beneficiaries. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their
needs and ultimately facilitate the improvement of their housing situation.The off-site temporary and/or emergency residential housing facility will be located within the Salem, Virginia community or within a 25- mile radius of the Salem VA and/or a 25-mile radius of the Community Based Outpatient Clinic (CBOC) and/or Health Care Centers (HCC). The off-site temporary and/or emergency residential housing facility shall have the capacity of fifteen (15) beds. The contracted facility must have a current occupancy permit or license required by the authority having jurisdiction.The Contractor shall provide Off-site Temporary Residential Housing Shelter services to include but not limited to beds, ADA accessible restrooms and bath, kitchen facilities, and related furnishings. Adequate security measures in accordance with VA standards and in conformance with state and local safety and health codes as outlined in 38 CFR, Section 17.53 to provide optimum safety for residents.Contractors shall provide food for two (2) nutritionally adequate and balanced meals daily and snacks daily to each shelter resident, as well as personal hygiene items, first aid supplies and storage, as needed.
