As a part of that commitment, and in agreement with standards set by the Public Health Accreditation Board, a Community Health Needs Assessment (CHNA) is conducted every three years. The CHNA provides essential information on the needs, strengths, and challenges our communities face. It informs resource allocation and strategies that directly address health disparities and community needs. Similarly, the Patient Protection and Affordable Care Act requires charitable hospitals to conduct a CHNA and adopt implementation strategies to meet identified needs every three years. This is the basis for the joint effort between MCDPH, Health Improvement Partnership of
Maricopa County (HIPMC), a collaborative of over 100 private and public organizations, and Synapse, a coalition of non-profit and federally qualified health care providers. This short video will give you more information about why the CHNA is important and how the community can participate. MCDPH is also tasked with addressing emergent issues such as the COVID-19 pandemic. COVID-19 has had a disproportionately negative impact on many of Maricopa County residents. It became evident early in the pandemic that health disparities and lack of health equity created greater challenges to prevention, mitigation, response, support to navigate care, address social isolation, and emergency response preparedness. This made many in our community much more vulnerable to the disease, socio-economic impact, and social isolation that continues to effect recovery for all. As such, there will be an increased allocation of funds to support additional assessments and opportunities to hear from the historically marginalized communities. The goal is to better understand the impact of COVID-19 on Maricopa County community regions, service providers, and individual residents. Data collected will aid in identifying priority health areas and barriers in Maricopa County and the target service areas for local hospitals and Federally Qualified Heath Centers (FQHC). The Office of Community Empowerment (OCE) will implement the CHNA in 2023 with an official launch date of March 1, 2023. CHNA survey implementation will encompass: - Maricopa Countywide outreach - Population focused outreach and engagement - Regional focused outreach and engagement - Electronic, paper, virtual and phone support to complete surveying - Multiple languages to ensure participation across priority populations - Community event participation Through funding provided by the Centers for Disease Control (CDC) to address health disparities that were either created or worsened by the COVID-19 pandemic, MCDPH is soliciting responses to increase CHNA response rates for specific subpopulations in Maricopa County as part of the Maricopa County CHNA. The information gathered will provide necessary insight into identifying emergent health needs, addressing barriers, determining resource allocation, and long-term COVID-19 response. MCDPH is interested in contracting with agencies who have a direct relationship to at least one of the following populations: - Racial and ethnic groups specifically: Black/African American Native American Hispanic Native Hawaiian/Pacific Islander Asian - Individuals who fall within the following age brackets: 12-18 65+ - Unsheltered - Disabled - Veterans - LGBTQ+ - Refugee, Immigrant, Migrant - Formerly Incarcerated - People living in rural areas of Maricopa County We anticipate the following timeline of activities: - January 2023: Contracts awarded and executed - February 2023: On February 2, 2023 and again on February 9, 2023, from 10:00am - 12:00pm, MCDPH will provide a mandatory in person group training/orientation for all agencies awarded a contract. Agencies (Contractors) are required to attend at least one of these sessions. In addition, MCDPH staff will work collaboratively with individual Contractors to develop a CHNA Implementation plan for their proposed focus population and/or focus region. - March 2023: Official launch of CHNA survey - March 2023 - May 2023: Survey open. MCDPH staff and Contractors work to engage the community and ensure all voices are represented. - June – December 2023: MCDPH staff will conduct data analysis in preparation to release survey results. MCDPH reserves the right to award to multiple Contractors and to add additional Contractors, at the County’s sole discretion, in cases where the currently listed Contractors are of an insufficient number or skill set to satisfy the County’s needs. MCDPH reserves the right to award to those Contractors most advantageous to the County. Awards are contingent upon funding. BID SUBMISSION: Vendor Bids must be submitted through PeriscopeS2G only.
