Page 1 of 4 PURCHASING DIVISION Calvin Brinks, Manager (616) 632-7720 purchasing@kentcountymi.gov SOLICITATION Solicitation Type Request for Proposal (RFP) Solicitation Number 5473 Description Strategies to Advance Health Equity - Kent County Health Department Date of Issuance 12/1/2023 Inquiries Deadline Date & Time (local) 12/13/2023 Due Date & Time (local) 12/20/2023 at 2pm Pre-Submission Conference No Buyer Name Brinks Needs Statement The Kent County Health Equity Council is made up of staff from community-based organizations and community members representing the diverse populations who call Kent County home, including racial and ethnic groups and those living in zip codes where social and economic factors limit opportunities to achieve optimal health. During the past year, Council members have engaged in honest conversations about the health concerns facing their communities and the factors that negatively impact
their well- being. Based upon feedback obtained through surveys and focus groups with the represented communities, the Health Equity Council is currently seeking to fund proposals to address the following objectives: 1) Increase positive interactions between police and community, 2) Increase feelings of safety among K-12 students, 3) Reduce mental health stigma and improve access to mental health support services, 4) Increase access to safe housing options, 5) Reduce inequities in access to affordable healthy food options, 6) Reduce linguistic and cultural barriers to improve access to medical care and 7) Reduce barriers to navigating the health insurance system. Note: This is a grant-funded initiative and project activities must be completed by April 30, 2024. Priority will be given to projects carried out by collaborative partnerships. Projects are capped at maximum $10,000 per project. Proposal Instructions Work Scope: Ensure the proposals include the following: Provide an overview of your organization and describe the population(s) you most often reach when providing services. If applicable, describe how your organization currently addresses the chosen area of focus Revised 12/12/23 Kent Page 2 of 4 What objective would your organization address with this funding and how would you use this funding to specifically address the chosen objective? What opportunities are there to partner with other organizations who serve a similar population to maximize the projects impact? How will you measure the success of your program, initiative, etc.? If incentives will be a part of the proposal, a detailed Incentive Plan is required. The Plan must include details related to eligibility determination and purchasing, tracking, and disbursement processes and be approved by Health Department Finance, if proposal is awarded. If you have an existing agreement with the Kent County Health Department to support staffing, you may not include that staffing in the mini-grant proposal unless prior approval is received from the Health Department. Budget: Provide a brief program budget that outlines the budget as follows (food is not an allowable expense): Amount Requested: $ Expense: Category Program Amount Staff (inclusive of benefits, taxes, etc.) Other (please describe) Total Length: Proposal (scope + budget) shall be no longer than three pages (size 12 font) in a .pdf version with pages numbered. Proposal Review The Proposal Review Team will make its allocation decisions based on the following rating criteria: The quality of the applicants approach to addressing one or more of the objectives identified by the Health Equity Council The potential for the proposed activities to impact racial and ethnic populations, individuals living in zip codes where social and economic factors limit opportunities to achieve optimal health, or those facing health inequities due to disability, sexual orientation or gender identity The feasibility of meaningfully addressing the chosen objective in the timeframe provided The efficacy of the applicants collaborative approach to addressing the chosen objective The quality and feasibility of the applicants approach to measuring success Review members will include the following: Members of the Health Equity Council who are not affiliated with an organization applying for funding Staff from the Kent County Health Department Center for Community Health Strategy and Health Department Finance. Kent County Purchasing Division Page 3 of 4 Expectations of Funding Recipients Participate in monthly meetings with KCHD and Health Equity Council representatives to provide updates on project successes and challenges Provide a presentation summarizing the project accomplishments at a Health Equity Council meeting or annual Health Equity Council Conference Awardees will be required to submit a detailed budget, including staff name, title, and salary and fringe rate to be approved by Health Department Finance. The detailed budget will be used to create an invoice template for reimbursement requests. A detailed workplan that includes objectives, activities, and timelines. Timely notification of any changes to staffing, budget, or workplan. Provide report of population served, including number and demographics. Provide report of community outreach and education events conducted. Other Due to limited funding, no guarantees of continued funding at current levels or increases in funding can be made to past Current Unmet Needs funding recipients. Should the County of Kent have any questions regarding your proposal, we will contact you for clarification. Otherwise, award notifications will be sent out in May. The Kent County Standard Submission Terms posted on the Bid Opportunities page of the Kent County Purchasing Division website are incorporated by reference and shall be deemed to have the same force and effect as if fully set forth herein. Kent County reserves the right to determine response format or not respond, in its sole discretion. REQUEST FOR PROPOSAL SUBMISSION Complete submissions must be received in the Kent County Purchasing Division on or before the due date/time specified by the designated clock (local time). Late, faxed, or emailed submissions will NOT be considered. Submissions may be submitted electronically on the Bid Opportunities page of the Kent County Purchasing Division's website. Respondent must include a complete proposal as 1 non-password protected PDF document. The time required to upload a submission may vary. Respondent assumes all risks associated with electronic submission, including technological difficulties, and deems the County and its service provider harmless and without fault. Successful electronic submissions are confirmed via Respondent's email. Respondent shall view the link in the confirmation email to determine accuracy prior to due date/time.
