Amendment #1The purpose of this amendment is to update the scheduled hours of performance, unit of measure, and quantity of line items 0001 and 0002 in the base, and following proposed option years.  The changes for each line item are as follows:Line Item 0001:  Sunday / Weekday Catholic Mass and Patient Pastoral CareDuty hours are changed from “Sunday through Thursday, 0600 – 1300” to “Sunday through Thursday, 0600 – 1400”. The unit of measure is changed from “days” to “Months” to streamline validation and billing of services on a monthly basis. The quantity is changed from “252” to “12” to accommodate the 12 months of billing in a calendar year. Line Item 0002:  On-Call Services – Non-Duty Hours Non-duty hours are changed from “Sunday through Thursday, 1300 – 0600, Fridays, Saturdays & Holidays (24 Hr. Coverage)” to “Sunday through Thursday, 1400 – 0600, Fridays, Saturdays & Holidays (24 Hr. Coverage)”.  Change to hours reflects the updates to hours of performance under line item
0001. The unit of measure is changed from “Hours” to “Months” to streamline validation and billing of services on a monthly basis. The quantity is changed from “6,990” to “12” to accommodate the 12 months of billing in a calendar year. *Note:  Performance of line item 0002 requires contractor to remain on stand-by to perform emergency visitation services outside of duty hours.  Historical performance of these services show an average of three (3) to four (4) “on-call” requests for emergency visitation during non-duty hours per week. Line items 0003 and 0004 remain unchanged.All changes made under this amendment are reflected on the attached updated CLIN Structure and Performance Work Statement.Original TextThis is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.  W912CN-20-Q-S0001 is issued as an invitation to Request for Quotation (RFQ).  The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05. This acquisition is a 100 percent small business set-aside.  The North American Industry Classification System (NAICS) Code for this acquisition is 813110 – “Religious Organizations”.  The size standard is 8.0 (millions of dollars).  It is the contractor’s responsibility to be familiar with the applicable clauses and provisions.    Description of Requirement:  The Department of Ministry and Pastoral Care at Tripler Army Medical Center, Honolulu, Hawaii, has a requirement for a Roman Catholic Priest to provide Catholic ministry and pastoral care, which includes daily patient pastoral care, Mass (Weekday, Weekend, Holy Day of Obligation as applicable), and on-call emergency services.  All job duties, personal qualifications, and endorsements must be performed in accordance with the attached performance work statement (PWS). All interested Priests must submit an application package for review to the Archbishop at the Archdiocese for the Military Services (AMS).  In accordance with the Office of the Under Secretary for Personnel and Readiness, the AMS is the sole official Department of Defense ecclesiastical authority for endorsement of Roman Catholic Priests seeking to contract directly with the Armed Forces.  Application items include at minimum: degrees and transcripts, endorsements, and experiences.  Interested Priests may visit the AMS website https://www.milarch.org/ and click “Forms” for the “Application for Endorsement / Faculties”.  Please note all applicants must have a myAMS account to access the AMS portal.   Statement of ecclesiastical endorsement is given on the DD Form 2088, a requirement item to be included with any RFQ response. The period of performance shall be for one base year of 12 months, and four 12-month option years.  The period of performance reads as follows:Base Year:  1 July 2020 to 30 June 2021Option Year One:  1 July 2021 to 30 June 2022Option Year Two:  1 July 2022 to 30 June 2023Option Year Three:  1 July 2023 to 30 June 2024Option Year Four:  1 July 2024 to 30 June 2025*The period of performance is subject to change depending on actual date of award.Location of Performance:  Services shall be provided at Tripler Army Medical Center at 1 Jarrett White Road, TAMC, HI 96859.  Contractor services are not limited strictly to patient and staff visitation areas, office areas, or the chapel, but ministrations may be performed in all areas of the hospital.Offeror must be registered in the SAM database to be considered for award.  Registration is free and can be completed on-line at http://www.sam.gov.Please see attached CLIN Structure for reference in submitting quotes.  Ensure pricing data is provided for all line items for base year, and follow-on option periods. Question Submission Deadline:  Offerors are requested to review this solicitation in its entirely, including attachments, and urged to submit any questions regarding this document to the Contracting Officer and Contract Specialist in writing via email (ryan.g.lee.civ@mail.mil;
