Request for Proposal Engineered Luxury Vinyl Tile Flooring for the Belknap County Nursing Home RFP# Flooring 2023 Requested by Belknap County Nursing Home 30 County Drive, Laconia NH 03246 (603) 527-5410 Instructions to Flooring Contractors 1. Proposals The Belknap County Nursing Home (BCNH) is requesting proposals for the purchase and installation of Engineered Luxury Vinyl Tile (ELVT) flooring and associated 4” cove base in several areas of the Nursing Home, and the removal and disposal of the old flooring and cove base in those areas. Sample boards of ELVT and cove base shall be provided to nursing home staff for selection of styles and colors. Submit proposal as shown: Flooring for the following areas is requested. However, measurement confirmation is to be field verified by vendor. Approximate square footage has been provided as a courtesy. 1. Measurements of the following areas: 2. Pricing for the purchase and installation of the ELVT flooring listing totals broken down by
manufacturer, square footage, price per square foot, and total price for the areas listed above. 3. Pricing for the removal and disposal of the existing flooring in the areas listed above. 4. Warranty on the installation and the flooring. 5. Manufacturer specification sheet on flooring submitted in the proposal. 6. Proposals shall be mailed or delivered so that it is received no later than 2:00 p.m., April 28, 2023. 2. Openings • Proposal packages are to be returned no later than 2 p.m. on April 28, 2013. • Envelope to be marked: RFP# Flooring 2023 • Proposal will not be accepted after the above designated date and time. Proposers may withdraw their proposals between the time they are submitted and opened, if desired. • Proposals will be opened and read aloud on April 28, 2023 at 2:15 p.m. 3. Notice of Award The contract shall be deemed as having been awarded when the formal notice of acceptance of contractor’s proposal has been duly served upon that contractor by an authorized officer or agent of BCNH. Unsuccessful contractors will be notified in writing. Successful contractor will be subject to COVID-19 protocols as per Centers for Medicare and Medicaid Services regulations (ex. Required to wear masks, etc.). 4. Proposal Prices Proposal prices are to remain in effect for a period of forty-five (45) days from opening date of the proposals and are to remain firm after award. 5. Discrepancies and Addenda Should a contractor find any discrepancies in the specifications contained within the RFP, or should the contractor be in doubt as to their meaning, the contractor shall notify BCNH at once who will send a written addendum, if necessary, to all contractors concerned. Oral instructions or decisions, unless confirmed by Addenda, will not be considered valid, legal, or binding. No extras will be authorized because of failure of the contractor to include work called for in the Addenda in their proposal. 7. Competency of the Contractor To enable BCNH to evaluate competency and financial responsibility of the Flooring Contractors, the successful contractor shall, when requested by BCNH, furnish the following information, which shall be sworn to under oath by him/her or a properly authorized representative. 1. The address and description of the contractor’s place of business. 2. The name and/or articles of co-partnership or incorporation. 3. A certified or authenticated financial statement dated within 60 days prior to the opening of the proposal. BCNH may require that any items of such statement be further verified. 4. A list of present installations including dollar values, and percentage of completion. In addition, names of all owners involved and separate list of trade references. 5. A list of similar contracts that have been completed in the past two years. 6. A statement regarding any past, present, or pending litigation with an owner. 7. Such additional information as may be required that will satisfy the BCNH that the contractor is adequately prepared in technical experience, or otherwise to fulfill the contract. 8. Sufficient documents to ensure that the flooring contractor is in compliance with the current fair employment practice requirements of the County and State. 8. Scope of Work Removal and disposal of current flooring and cove base including delivery and installation of the new ELVT flooring in the Center Corridor, Service Corridor, East Day Room, East E-104 Corridor, East E-2 Corridor, Nurses Station East, East Short Hall, East Kitchenette, East Corridor E-4, East Employee Entrance, West Day Room, West @-6 Corridor, Nurses Station West, West Short Hall, West kitchenette, West Corridor W-8, West W-104 Corridor, Ambulance Entrance, Laundry, Vestibule by Courtyard, Activities Room, Front Entry, Nook, Meditation Room, Family Room, Gym and Hair Salon. This shall include any skimming or trimming to provide smooth transition from the new ELVT flooring to the existing flooring in the facility and/or any additional material to make a seamless transition between the new and old flooring. All necessary equipment required to meet the intent of these specifications, whether or not enumerated within these specifications, shall be supplied to provide proper adhesion and installation. Floor prep and installation shall be performed in accordance with manufacturer's guidelines. Areas should be patched and leveled where needed to promote a smooth, level, and uniform finish. Floor leveling and patching materials should be used that do not interfere or interact with cleaners, glues, primers, or adhesives. 9. Cleaning and Patching It shall be the responsibility of the Contractor to keep their work area clear of debris and to clean the area after the installation is complete. At no time shall equipment or materials be left unattended. It shall be the responsibility of the Contractor to patch any wall or surface that has been disturbed by the execution of this work, however painting shall be the responsibility of BCNH. Any and all penetrations of the fire walls must be filled in with appropriate material to meet or exceed a two hour fire rating per NFPA 101 Life Safety Code, 2012 edition, as referenced for long term care facilities 42 CFR Part 483.70(a). Any and all other penetrations of any walls above or below the ceiling must be filled with appropriate material to withstand smoke barrier construction. 10. Schedule All work to be done shall be scheduled through the office of the Environmental Services Director. It is the responsibility of the Contractor to coordinate all work with the other trades for scheduling rough-in, and finishing all work specified. BCNH will not be liable for any additional costs due to missed dates or poor coordination of the Contractor with other trades. The installation is to take place in an occupied nursing home. At all times the safety and comfort of the residents is paramount. That said, the transition from old to new shall be seamless, and there shall be minimal down time. Any down time shall be scheduled with and approved by the Administrator. 11. Warranty The Contractor shall provide a warranty for the installation of the material and the warranty information for the selected flooring. 12. Maintenance Contractor will provide specifications and or instructions for initial and routine maintenance of the flooring as well as any specification for initial and routing cleaning. 13. Disqualifications Any one or more of the following causes may be considered sufficient for the disqualification of a Contractor and the rejection of his/her proposal. 1. Failure to submit proposal on time. 2. Evidence of collusion among contractors. 3. Lack of responsibility as revealed by either financial, experience or equipment statements, as submitted. 4. Lack of expertise as shown by past work, and judged from the standpoint of workmanship and performance history. 5. Uncompleted work under other contracts which, in the judgment of the BCNH, might hinder or prevent the prompt completion of additional work if awarded. 6. Being in arrears on existing contracts, in litigation with an Owner, or having defaulted on a previous contract. 14. Payment The Contractor shall submit invoices to BCNH for approval and processing at the completion. Contractor is solely responsible for withholding taxes for its employees. 15. Acknowledgements In submitting this proposal, the Contractor acknowledges the following: Acceptance: BCNH reserves the right to accept or reject any proposal for any reason or no reason without recourse by the Contractor to award a contract to the Contractor on any basis which BCNH, in its sole and absolute discretion, determined to be in the best interest of BCNH. The county reserves the right to add and deduct areas from the aforementioned list of areas based on the availability of funding for the project. The BCNH intends on replacing the flooring and cove base in all areas but understands that costs of materials, market demands, and availability of contractors may limit the total scope of the project. 16. Indemnification To the fullest extent permitted by law, Contractor shall protect, indemnify, save, defend and hold harmless the County of Belknap, including its officers, officials, volunteers, employees and agents, from and against any and all liabilities, obligations, claims, damages, penalties, causes of action, costs and expenses, including reasonable attorney and paralegal fees, which County of Belknap and/or its officers, officials, volunteers, employees and agents may become obligated by reason of any accident, bodily injury, personal injury, death of person, or loss of or damage to property, arising indirectly or directly in connection with or under, or as a result of this agreement, but only to the extent caused in whole or in part by any negligent or wrongful act or omission of contractor and/or its officers, members, directors, volunteers, employees, subcontractors and/or agents. 17. Insurance Coverage The Contractor shall maintain at all times during the life of this contract the following insurance coverage. The Contractor must also require its subcontractors to maintain such coverage. Workers Compensation Insurance: The Contractor shall carry workers compensation insurance as required by the State of New Hampshire. Comprehensive General Liability Insurance: The Contractor shall maintain a broad form comprehensive general liability insurance policy in an amount of no less than $1,000,000 combined single limit per occurrence. Motor Vehicle Insurance: The Contractor shall carry motor vehicle insurance to include bodily injury, property damage, uninsured motorist, and employer’s non-ownership coverage in an amount of no less than $1,000,000 combined single limit per occurrence. Belknap County must be named as an additional insured on the Comprehensive General Liability Insurance and the Motor Vehicle Insurance policies required by the foregoing. Contractor shall provide an insurance certificate confirming the above insurance coverage and such endorsement. The insurance certificate and the underlying insurance coverage shall be issued by the carrier authorized to do business in the State of New Hampshire and having A.M. Best Company rating of “A” or better. The Contractor shall have professional insurance/errors and omissions insurance with limits of not less than $1,000,000 each occurrence, where applicable. The Contractor shall file certificates with Belknap County showing proof of insurance at the time the contract is awarded. A 30-day notice is required for cancellation and /or material change of coverage, sent directly to the Belknap County Commissioners Office at 34 County Drive, Laconia, N.H. 03246 18. Contract Price Contractor may submit proposal on company forms but RFP must be completed in full
