Header Comments
Contractor shall deliver "as needed" Services at the rates and within the amounts authorized under Part 2 of this Exhibit A. "As needed" Services will be agreed upon in a Work Order (WO) issued under this Contract in accordance with Section 4 of this Exhibit A, Part 1.

Contractor shall maintain COREMETS application and provide 1095B, 1095A and financial data query support through the following types of Services through approved Work Orders.

a. Maintenance & Operations: Server, portal and database
administration support as needed;
b. 1095A Queries: Assign access rights on request, provide help on portal issues;
c. Financial Data Queries: Specific reporting or report generation through manual SQL queries;
d. 1095B Queries: Assign access rights on request, provide help on portal issues; and

e. Assist in archiving database as needed.
(1) Maintenance & Operations
Contractor shall maintain COREMETS with the following services as needed:
a) Applying application patches and updates as recommended by the application vendors.
b) End user email support for questions regarding system access or availability.
c) Regular application and data backups, including the financial data schema.
d) Troubleshooting reported defects and proposing solutions.
Application hosting is provided by DAS Enterprise Information Services (EIS) Data Center Services (DCS).
(2) 1095A queries
a) Contractor shall assign or delete user rights to CORE as requested by OHA Contract Administrator. DCBS and DHS requests will go to OHA Contract Administrator for approval then to Contractor.
b) Contractor shall maintain portal access to application as requested by OHA.
(3) Financial data queries
a) OHA requests for specific financial data will be as detailed and descriptive as possible, Contractor shall clarify the scope with OHA and shall perform the SQL queries and compile the data needed.
b) OHA will determine the report format and method of delivery if the data is sensitive, Contractor shall produce the report and forward to OHA in the manner specified.
(4) 1095B queries
a) Contractor shall assign or delete user rights to METS as requested by OHA Contract Administrator. DHS requests will go to OHA Contract Administrator for approval then to Contractor.
b) Contractor shall maintain portal access to application as requested by OHA.
(5) Archive Database
At its sole discretion, OHA may request assistance in archiving the COREMETS database in a form and format to be determined at the time of the request and Contractor shall provide the archival Services at the time agreed upon by the parties, but no later than the termination of this Contract.

Work Order Process, Review and Approval
a. Upon OHA's requests of Contractor to deliver "as needed" Services under this Contract, OHA and Contractor will develop a written Work Order request in the form of Attachment 1 of this Exhibit A.
b. OHA's Contract Administrator and Contractors' Authorized Representative or their delegates will identify the Services to be completed by Contractor.
c. OHA's Contract Administrator and Office of Contracts and Procurement must review all Work Orders and any expenditure of funds before Contractor commences Services pursuant to such Work Order.
d. The Work Order must be signed by both parties to be effective and a Notice-to-Proceed (NTP) sent to Contractor by OHA (email is acceptable).
e. A Work Order at a minimum must contain the following information:
(1) A detailed description of the Services to be provided under the Work Order, including minimum acceptance criteria documenting that the Services were provided as specified;
(2) An agreed upon start and end date for Services; and
(3) The cost of the Services to be provided pursuant to the Work Order.
f. Cumulative Work Orders for Services shall not exceed $140,000.
