The purpose of this amendment is to set up a site visit at Lee County VA Hospital on 23 May 2022 at 10:00 am. This amendment also extends due date for quotes to 27 May 2022 16:00 and provides responses to contractor questions and floor plan drawings. RESPONSES: Is there any known presence of asbestos and/or mold for any of the facilities to be serviced? No asbestos, Cape Coral VA is a new building that opened Dec. 2012 No known mold currently 2. Can you confirm the Bonding requirements for this project? - no Bonding 3. Is there an incumbent for this opportunity? - previous contract ended 4. Will there be an opportunity for a site visit to view/assess the facility and the areas for servicing? Yes, tour as established/scheduled by contracting officer 5. What is the estimated population utilizing each facility/area for service? Daily 1,100 patients, family, caregivers, and visitors with approximately 475 staff 6. As this is a legally binding document, will you be providing an

Are there ample dedicated secure Janitorial closets and/or storage space(s) provided for the storing of cleaning equipment and supplies onsite? Yes, 14 housekeeping closets, machine room , 5 storage rooms, 9 linen rooms 16. Based on the cost of doing business during and beyond the COVID-19 pandemic, will additional consideration be given, (based on the lowest priced), in the evaluated purchase price? - Yes 17. Can you confirm that the successful bidder will provide all labor, uniforms, equipment, and all supplies except for the paper, toilet tissue, and hand soap? Labor, safety shoes, & Uniforms only. All equipment and supplies will be provided by the VA. NOTE: No Chemical or equipment is to be introduced into the facility without concurrence of the COR or their direct representative in his absence. 18. Will the contractor be responsible for providing consumables (paper [hand towels, toilet paper, toilet seat covers, etc.], hand soap, sanitary materials, and plastic [trash can liners])? NO 19. Based on the Performance Work Standard [PWS], equipment such as computer equipment, calculators, and similar instruments on or in the furniture shall not be dusted, is this correct? Yes, to computers and medical devices No to lobby kiosks 20. Based on the current Performance Work Standard [PWS], are there any new (and/or additional) service requirements that are included in this proposed PWS? -? Read the PWS? 21. Solicitation indicates that Quotes are due by 12:00 P.M. EST on May 20, 2022; SAM.Gov header page indicates Proposals are due by May 10, 2022. Can the VA please clarify the proposal due date, and questions due date? Contracting Officer needs to respond 22. In the case of off site relocation of some operations will the government consider ODC in operational expenses (no limit on distance or time required to travel or transporting of equipment and supplies for off site) No Offsite locations 23. The VA indicates that total disinfection cleaning in treatment and exam rooms, shall be accomplished daily. These services shall be provided Monday through Friday excluding Federal holidays at all clinics; NOTE: Does this mean that these exam rooms are not to receive disinfectant service after each patient visit? Nursing/providers disinfect after patient visits; Housekeepers will provide terminal cleaning as requested by clinical staff. 24. The VA states that "All exterior windows shall be cleaned at least monthly" Would the VA please provide the height and quantity of the highest windows from ground level to the window top surface? Not to exceed 10 feet height as reached with VA provided equipment; 185 windows 25. The list shall be provided not later than the __________work day of the following quarter. Can the VA provide "which work day of the following quarter" it is referring to for this requirement? 10th workday 26. Can the VA provide the number of fixtures, stalls, urines per bathroom, or provide the floor plans showing layout? Floor Plans attached 27. Would the VA please consistently number the pages and paragraphs of the Solicitation and PWS so that references to specific paragraphs and sections can be ascertained Contracting Officer 28. What are the required certifications due at submission? Proof of company or individual employee certifications & training. i.e. HAI; terminal cleaning for bodily fluids, C-diff, COVID, MRSA, Scabies, Flu, Etc.; refuge & Bio hazardous material handling; Donning/doffing PPE; lifting ergonomics; sight, hearing, steel toe protection; 29. Does the Shift Leader require any kind of certifications as denoted in 1.2.3.1? Experience as verified by the Contractor Hiring Agent 30. The government specifies that "services shall normally be accomplished during normal working hours." Would you please confirm the standard hours the clinic will be open. - Technical Exhibit A states - Note: Bay Pines VA Healthcare System has expanded clinic hours for Lee County. The clinic will remain open until 7:00pm on Wednesday nights and will be open from 8:00am to 1630 on Saturdays. Contractor shall provide continued housekeeping services to cover expanded clinic hours. Housekeeping staff cleans the facility from 6 a.m. until midnight Monday thru Friday. Floorcare is accomplished after normal working hours on a not-to-interfere basis with patient care seven days a week and on holidays. 31. What is the height of the ceilings? Various exam rooms 9 feet, Lobbies 9 feet, main lobby 25 feet, canteen 12 -25 feet, conference room 20 feet 32. Will the Government provide a list of the disinfectant and cleaning chemicals? - Yes
