This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issuedGENERAL: The Phoenix Indian Medical Center currently has a need for annual preventive maintenance, repairs, parts replacement (as required), labor and travel for Hydrogen Peroxide Sterilizers used in Central Sterile Room. The agreement shall be a full service. The medical equipment is located at the Phoenix Indian Medical Center (PIMC) in Phoenix, Arizona. The PIMC is located at the address below: 1.1 Phoenix Indian Medical Center 4212 N. 16th Street Phoenix, Arizona 850162. SCOPE OF WORK: The contractor shall provide all supervision, labor, parts, materials, tools, supplies, maintenance, ground transportation,
and equipment service for Hydrogen Peroxide Sterilizers. 2.1 Contractor shall repair or replace all components necessary to return units to Original Equipment Manufacturer’s specifications.2.1 Contractor shall warrant all services performed and units shall remain defect free 30 days from the date of service/maintenance/repair acceptance.EQUIPMENT:1Small Hydrogen Peroxide SterilizerSN: 0033101626Model: Sterrad NX2Large Hydrogen Peroxide SterilizerSN:101120260Model: Sterrad 100S4. SERVICE DESCRIPTION: The contractor shall provide supervision, labor, replace parts, materials, tools, supplies, maintenances, and repairs. The service shall include certifications, calibrations/calibration verifications, replace parts (as required), and provide service reports on the items in SOW Section 3 above.4.1 The contractor shall be authorized by manufacturer to perform all work included in the SOW and supporting documents and shall obtain and maintain all required manufacturer, Federal, State, and local licenses and certifications associated with the performance of the services.4.2 Performance shall be according to the SOW, supporting documents and shall also conform with:4.2.1. Original Equipment Manufacturers (OEM) specification requirements4.2.2. National Fire Protection Association (NFPA) 99 and 1014.2.3. Standards for Health Care Facilities4.2.4. Environment of Care (EC.02.04.01, EC.02.04.03)4.2.5. The Joint Commission standards4.2.6. Federal, Tribal, State and local regulatory guidance shall be applied in regards to bio hazardous waste, disposal, and applications. 4.3 Work shall be performed in a hospital environment. Contractor shall comply with HIPAA security requirements for safeguarding patient information.4.4 The contractor shall have ready access to new OEM parts. All parts replaced shall be new OEM.
