COUNTY OF AMADOR REQUEST FOR PROPOSALS 22-25 PUBLIC HEALTH AFTER-ACTION REPORT FOR THE COVID-19 PANDEMIC PROJECT DESCRIPTION SUMMARY Amador County Public Health Department is requesting proposals from qualified consultants, contractors, respondents, or firms to assist with the department’s After-Action Report for the COVID-19 pandemic. An After-Action Report will help the Public Health Department analyze the management or response to the COVID-19 Emergency incident by identifying strengths to be maintained and built upon, as well as identifying potential areas of improvement. It is anticipated that one firm will be selected to perform all services related to Public Health After-Action Report (AAR) for the COVID-19 pandemic in collaboration with Amador County Public Health. Qualified bidders may acquire a bid package at the office of General Services Administration located at 12200-B Airport Road, Martell, CA, 95654 telephone (209) 223-6375 between the hours of 9:00 a.m. and 4:45 p.m. Monday through Friday ; or downloaded from Public Purchase website at www.publicpurchase.com

Sealed bids will be received by the County of Amador Purchasing Office; by U.S. Mail at 12200-B Airport Road, Jackson, CA 95642-9527; by U.P.S., Federal Express, or by another courier to 12200-B Airport Road, Martell, CA; 95654, until 1:30 PM, Thursday, December 22, 2022 , at which time sealed bids will be opened and read in the office of the Director of General Services Administration, located at the above Martell address. Bids received late, will be rejected and returned unopened. Telephone, Facsimile (FAX), and E-Mail bids will not be considered.
