STATEMENT OF WORK LEROY POND ADA DOORS VETERANS HEALTHCARE SYSTEM OF OZARKS BACKGROUND Contractor to provide and install six automatic door openers on two single walkway doors and two sets of double entry doors to allow easy access to the facility for handicaped patients and visitors. SCOPE AND DEFINITION OF SERVICES Contractor shall provide all labor, tools, equipment and transportation and supervision necessary to complete all work associated with installing automatic door openers on two separate single entrance doors at Building # 30 and two sets of double entrance doors at Building # 32, located at Leroy Pond / Veterans Healthcare System of the Ozarks (VHSO) located at 1100 North College Avenue, Fayetteville Arkansas 72703. OVERVIEW VHSO owns and/or operates Leroy Pond which offers residential programs among other services to our veterans. The purpose of this contract is to ensure; z All equipment shall be maintained to operate in a correct and safe manner as intended and designed

by the Original Equipment Manufacturer (OEM). Preventive maintenance is completed on time and in accordance with OEM recommendations. All software operates as designed. All equipment/items are inspected to ensure safe and correct operation in compliance with applicable local, state and federal regulations. Equipment downtime is kept to a minimum by maintaining no less than a 98% in-service rate for all covered equipment.
