SCOPE OF WORK St. Joseph's Healthcare Hamilton (SJHH) is the regional healthcare provider for dialysis services. The current

dialysis water purification system and distribution loop at the outpatient unit of SJHH’s King Street campus is requiring replacement. The Successful Proponent shall replace the existing reverse osmosis (RO) water purification system, pre-treatment system and sanitary distribution system loop with a turnkey dialysis water purification plant and sanitary distribution system for 62 (sixty-two) points of use, as well as installing and an additional 2 (two) more. One point of use will be installed in Exam Room 3 and Exam Room 4 as per Appendix: A Dialysis Unit Floor Plan and Loop Layout and Appendix E: Exam Rooms 3 and 4 - New Installations. The ultrapure permeate water shall meet the Canadian Standard Association (CSA) CAN_CSA/ISO Z23500-2:20 and Z23500-3:20. Furthermore, an interim system proposal must be included with the proposal. This interim system must meet the aforementioned standard for ultrapure permeate and shall be adequate to operate at minimum 50 percent of the dialysis points of use while the new system is being installed and validated. Subject to SJHH’s reserved rights and privileges set out in the Proposal, the Contract shall be awarded to the compliant Proponent who has the highest overall Total Evaluation Score, and has met or exceeded all minimum required points per category (if any) and the Benchmark. This RFP is open to all suppliers. All responses to this Request for Proposal shall be submitted in English. 2. SITE MEETINGS Mandatory Site Meeting There is a mandatory site meeting scheduled. Failure of a Proponent to attend this site meeting will result in the rejection of any Proposal submission by that Proponent. SITE VISIT ATTENDEES MANDATORY COVID-19 VACCINATION: St. Joseph’s Healthcare Hamilton has introduced a COVID-19 Immunization Policy that is aligned with regional, provincial, and federal recommendations regarding infection prevention and screening. Effective December 14, 2021, the policy requires all Contractors working on-site at SJHH to be fully vaccinated, (apart from approved medical exemptions) and be compliant with St. Joseph’s COVID-19 Immunization Policy. “Fully vaccinated” means having received the full series of a COVID-19 vaccine or combination of COVID-19 vaccines approved by Health Canada; and having received the final dose of the COVID-19 vaccine at least 14 days ago. Prior to attending the site meeting at SJHH, Contractors shall adhere to this policy and demonstrate their compliance by completing and submitting an Attestation Form, which can be found at: stjoes.ca/ContractorVaccinationAttestation Failure to complete the Attestation may result in access to SJHH premises being restricted.
