Personal Resume – (15% of group 1) Please attach your current resume in order to describe your experience you have assisting clients in the target population (adults with disabilities aged 18 and older, seniors aged 60 and older, veterans, and/or their caregivers living in Multnomah County) in a variety of geographical locations, including but not limited to client homes, adult foster care sites and hospitals. Identify your number of years experience and your assigned roles and responsibilities. 2.1.3 Q2.1.3. Narrative Writing Sample - (10% of group 1) Please attach a Narrative Intervention Writing Sample, as "Intervention Sample". The purpose of this writing sample is to narrate a past intervention you were involved in to help stabilize an individual within the target population. Provide a summary of a client situation, identifying complicating factors, persons involved, solutions offered and outcomes. Do not provide any client or agency specific information that would identify the consumer, agencies, etc. Limit your response to not more than two (2) pages, single sided for this narrative. 2.1.4 Q2.1.4 Community Resources – (20% of group 1) Please describe your knowledge and experience utilizing local community resources within the Portland Metropolitan area and the local aging and disability services system.
