Identify your number of years experience and your assigned roles and responsibilities. 2.1.3 Q2.1.3. Narrative Writing Sample - (10% of group 1) Please attach a Narrative Intervention Writing Sample, as "Intervention Sample". The purpose of this writing sample is to narrate a past intervention you were involved in to help stabilize an individual within the target population. Provide a summary of a client situation, identifying complicating factors, persons involved, solutions offered and outcomes. Do not provide any client or agency specific information that would identify the consumer, agencies, etc. Limit your response to not more than two (2) pages, single sided for this narrative. 2.1.4 Q2.1.4 Community Resources – (20% of group 1) Please describe your knowledge and experience utilizing local community resources within the Portland Metropolitan area and the local aging and disability services system.
