Excerpts from the CFY-BCC Application Guidance:

1.01Purpose

The purpose of this Application Guidance is to provide eligible
applicants information on completion of the application for the FY19 Care for Yourself (CFY) - Breast and Cervical Cancer Screening Program. Services covered by this application include financial support to local boards of health to assure the provision of outreach, program enrollment, breast and cervical cancer screenings and diagnostic services, data reporting, and patient navigation services for eligible persons.   
 
1.02Project and Contract Period

Funding during the defined project period is dependent on approval of the Application, contractor performance during the fiscal year, compliance with general and special conditions of the contract, availability of project funds, program modifications, or any other grounds determined by the Department to be in the Department’s best interest. The Department expects the contract period to be a one-year term from June 30, 2018 to June 29, 2019.
 
The issuance of this Application Guidance in no way constitutes a commitment by the Department to award a contract.

1.03 Eligible Applicants

Applicants must meet each of the following eligibility requirements for consideration.
 
Eligible Applicants

Local Boards of Health (LBOH) are listed in the FY19 Funding Allocation Table (see below) and will be the only entity eligible to apply for and serve as the contractor for the FY19 Care for Yourself (CFY) - Breast and Cervical Cancer Screening Program contract. Local Boards of Health (LBOH) have jurisdiction over public health matters in their designated geographic area. A Local Public Health Agency may not apply in response to this posting.
 


FY19 Funding Allocation Table Key:  

1-      Available funds for BCC Screening Services (includes Komen and Patient Navigation)

2-      Overall Number to be screened (includes Komen and Patient Navigation in overall total)

3-      Number of Komen Women to be screened (included in screening total)

4-      Number of Patient Navigation Services (included in screening total)

5-      Available funds for Outreach Services

6-      Available funds for High Priority Komen Outreach (8 Selected areas)

7-      Total Available funds (Includes BCC Screening Services, Outreach Services & High Priority Komen Outreach Funds)
 
Electronic Communication Requirements

Applicant is required to maintain and provide to the Department, upon application, a current and valid email account for electronic communications with the Department.
 
Official email communication from the Department regarding this application will be issued fromiowa.grants@webgrantsmail.com. Applicants are required to assure these communications are received and responded to accordingly.

1.04Service Delivery Area

Service Delivery area is the county represented by the eligible applicant.
 
Refer to the Funding Allocation Table posted in Section 1.03 of this Funding Opportunity posting. Any service delivery area changes for this application process must be agreed to by existing contractors and applicable boards of health, and approved by the Department. Submit requests to (Lindsey Jones at Lindsey.Jones@idph.iowa.gov). Written approval must be obtained prior to submitting an application.  

1.05Funding

The source of funding includes a federal cooperative agreement, state appropriations, and contracts with the Susan G. Komen of Greater Iowa affiliate.
 
Applicants may apply for the allocated funding amount as designated per program service area for the contract period within the FY19 Projected Funding Allocation Table (Section 1.03).
 
Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Department’s best interests.
