Psychosocial Support Services for Choices for Care and MFP Participants Residing In or Transitioning to Community

To develop a program of psychosocial support services for Choices for Care (CFC) and MFP participants currently in community-based settings, as well as those transitioning from Skilled Nursing Facilities (SNF’s) to community-based settings, that will enhance on-going self-care and address social isolation of both participants and their caregivers. This will be done through contracting with up to 2 entities experienced with the use of Elder Care Clinicians to provide services and supports.
