This a multiple award solicitation and one vendor may be awarded for each category detailed in the SOW. A more complete description of the services to be provided is in the Questions section. Group 2: Scope of Work Instructions: 2.1 Offeror must download, review and prepare a response in their template using the content and outline in this attachment. The completed document should be included in this question, and any additional attachments should be clearly named and uploaded to the Supplier Attachment section of this event. References must be related to the scope of services for which the agency is bidding. Provide the company name, dates of service, contact person, contact person title, contact telephone number and e-mail address, and a complete description of the services provided. References may be contacted to verify agency’s ability to perform the contract. The State reserves the right to use any information or additional references deemed necessary to establish the ability of the offeror to perform the contract. Negative references may be grounds for proposal disqualification. Group 6: Confidential Information Instructions: 6.1 Does your proposal contain confidential information? If you select Yes, another question will generate requiring an upload of your Trade Secret Affidavit. Offerors MUST also upload a redacted copy of their proposal under Supplier Attachments. 6.2 Offerors must upload a completed Trade Secret Affidavit Group 7: Verification of Nondiscrimination Against Firearms Entities/Trade Associations. Instructions: Offeror’s certification is made in compliance with and in reference to HB 356, Ch. 193, Mont. L. 2023 (HB 356) and the terms defined therein. If Offeror determines the provisions of HB 356 do not apply to Offeror or the contract to be awarded under this RFP, Offeror shall submit a statement setting forth in detail the basis for such determination. 7.1 a. Offeror does not have a practice, policy, guidance, or directive that discriminates against a firearm entity or firearm trade association during the term of any contract it is awarded under this solicitation; and b.
