The Lebanon VAMC is seeking SDVOSB/VOSB and any Small or Large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Need below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have given you permission to provide and install any systems or software.

Responses are due no later than Wednesday 2/17/2021 at 3:00 pm. This requirement is for the purchase, delivery, installation, of equipment as stated below (Brand Name or Equal) at the Lebanon VAMC. HAAG-STREIT SLIT LAMP AND TONOMETER Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for one Haag-Streit BX 900 LED Slit Lamps, with an AT 900 Tonometer to be utilized in an Optometry clinic setting. The Haag-Streit Slit Lamp will be used to facilitate Microscopy of the Eye. Scope of Work: The Slit Lamps, and coordinating accessories are the staple to all eye exams. This device is used to perform a Microscopy exam of the eye, which examines ocular health under high magnification, and measures intraocular pressure with the attached tonometer (screening and treating glaucoma). This is used for every patient that passes through the Optometry clinic, so it is used in high volume, and must be very durable and have a long-life span. Items Salient Characteristics of the Haag-Streit BX 900 LED SLIT LAMP: LED Illumination Head Light Source Eyepiece with double cross hair reticule Switch for beam splitter Diffusor Pivoting background illumination Imaging Module IM 900 Release Bar Illumination Control High precision mechanics built with high grade materials, ensuring decades long durability and longevity Salient Characteristics of the Goldmann Applanation Tonometer AT 900 Goldmann Applanation Tonometer Compatible with and attaches to the Haag-Streit BX 900 LED Slit Lamp Additional Information: Equal brands must the following characteristics Long lifecycle Tonometer prisms Interface ability with light bulbs are specific to the Slit Lamp Units do not have a delay when switching light and size settings Less space between the doctor and patient, making the unit more functional for people smaller in stature, as they are able to reach the patient behind the slit lamp. Delivery: The Haag-Streit Slit Lamp and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through Bio-Med, Optometry Chief and/or COR.
