Responses to the information requested will assist the Government in determining the appropriate acquisition method.The Great Lakes Acquisition Center (GLAC) - Southern Tier in North Chicago, IL is soliciting businesses for Digital Cable Television Service at three VA Hospital (Jesse Brown, Auburn Gresham CBOC, and Lakeside CBOC) The current systems and devices are listed in the SOW attached. It is understood that all Contractors submitting a response to this Sources Sought will have an established business with Five (5) years of consecutive experience in performing these types of services at healthcare facilities. The Contractor must also be licensed and authorized to perform
services on all the equipment listed.The Contractor shall furnish all labor, supervision, materials, tools, and equipment necessary to complete the required services. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 517311 Wired Telecommunications Carriers (e.g., with a Size Standard of $15 Million in annual revenue.Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. Contractor shall provide high quality picture and sound commercial television service to the Jesse Brown VA Medical Center campus, Auburn Gresham Community-Based Outpatient Clinic, and Lakeside Community-Based Outpatient Clinic in Chicago, IL.Specific Tasks/DeliverablesTask 1: Contractor shall provide commercial television service with a minimum of 35 television stations listed below (to include local channels) to all television sets and receivers at the Jesse Brown VA Medical Center campus. There are a total of 311 television sets along with 36 receivers located throughout the facility including the Bed Tower and Building 5.Task 2: Contractor shall provide commercial television service with a minimum of 35 television stations listed below (to include local channels) to all television sets and receivers at the Auburn Gresham CBOC. There are a total of 10 television sets (6 televisions with 3 receiver boxes in the 1st floor clinic and 4 television sets with 4 receiver boxes in the 2nd floor clinic area).Task 3: Contractor shall provide commercial television service with a minimum of 35 television stations listed below (to include local channels) to all television sets and receivers at the Lakeside CBOC. There are a total of 2 television sets with 2 receiver boxes.Task 4: Contractor shall conduct semi-annual site visits to each facility each year to ensure proper signals are being received and for quality of picture and sound. All service/repairs will be performed between 9:00am 3:00pm, Monday through Friday, unless requested and/or approved by the COR or designee.Holidays include:New Years' Day Labor DayMartin Luther King Day Columbus DayPresidents' Day Veterans' DayMemorial Day Thanksgiving DayIndependence Day Christmas DayAnd any other day specifically designated as a national holiday by the President of the United States.System Acceptance/TestingUpon completion the system will be tested by the COR along with the Service Provider Technician to ensure cable signal, sound and picture are performing at high quality and that all television receivers are receiving cable picture and sound upon satisfaction to end user.After the installation has been completed and it is found that cable television service is notperforming to desired levels in sound and picture quality the technician will troubleshoot problems until issues have been corrected and will maintain and or replace and defective equipment.Service InterruptionThe contractor shall provide a toll free number to speak with a Customer Service representative at any time, 24 hours per day, 7 days per week in the event of a service interruption defined as an interruption of service to which the cable signal is not being received due to technical difficulty which results in the cable broadcast being halted.Planned Interruption of Service: The Contractor shall provide the COR at least 24 hours of notice prior to any planned interruption of service.The contractor s customer service shall respond to all requests upon contact to troubleshoot any service interruption and/or dispatch a technician on site to troubleshoot and repair any issues of interruptions.The contractor s technician shall respond during the times of: 9:00am 3:00pm. The technician will troubleshoot and repair to completion to restore service if any interruptions occur.Credit for Loss of ServiceCredits for unintentional loss of service in excess of eight (8) continuous hours shall be granted as a pro-rated credit or rebate of the regular monthly charge to the end user.If during scheduled maintenance, repairs or updates there is a loss of service in excess of eight continuous hours the service provider shall grant the pro-rated credit or rebate of the monthly charge whether specifically asked for or not by the end user.GeneralThe COR Point of Contact will be Michael Randolph (Acting Chief Biomedical Engineering Service), located at the Jesse Brown VA Medical Center.Contractor will inspect all 3 facilities to determine equipment that is necessary to deliver cable and local television services to all TV sets at all 3 facilities.Contractor will provide cable receivers, coaxial cables, splitters, wiring and other equipment as required to ensure high quality cable service with high quality sound and picture.Customer shall have the flexibility to add additional CATV outlets as desired.
