This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-07, Effective Date: 08-31-2020. This solicitation is set-aside for Small Business. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219: Other Electronic and Precision Equipment Repair and Maintenance, with a small business size standard of $22 Million. The FSC/PSC is J065: Maintenance Repair of Equipment-Medical, Dental and
Veterinary and Maintenance. The Department of Veterans Affairs (VA), VA Caribbean Healthcare System (VACHS), San Juan; Puerto Rico requires preventive maintenance service, repair, and corrective maintenance for schedule Audiology Equipment listed in Attachment C, Scope of Work, attached to this solicitation announcement and located at the VACHS Medical Center in San Juan and his clinics in the Island of Puerto Rico. Contract Period: This solicitation contemplates a contract award that consist of a one year Base Period and four one year Option Period. A total of five year, sixty month of service. Quotation shall be submitted to Mr. Israel Rodriguez, via email at: Israel.rodriguez-perez@va.gov , on or before September 15, 2020 at 10:00 am, Eastern Time. Place of Performance/Place of Delivery: Department of Veterans Affairs VA Caribbean Healthcare System Facility Management System 10 Calle Casia, San Juan, PR 00921-3201 The full text of FAR provisions or clauses may be accessed electronically at http://acquisition.gov/comp/far/index.html. The following solicitation provisions apply to this acquisition: 1. FAR 52.212-1, Instructions to Offerors Commercial Items In accordance with FAR 13.106-2(b)(3) Evaluation of quotations. The Contracting offices will conduct comparative evaluations of quotation. Government will award a contract resulting from this solicitation to the responsible quoters whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: a. Company profile and capability statement comparable to the marketing package your company uses in their ordinary course of business. Please ensure the following information is included: point of contact information, DUNS Number, company business size and socioeconomic category such as: Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran- Owned Small Business (VOSB), Women-Owned Small Business, HUBZone Small Business, Small Business. b. Provide evidence/certification by manufacture to work with schedule equipment in the geographical area of Puerto Rico. c. Information of personnel record keeping insuring qualification technician is UpToDate. d. Past Performance Factor - An offeror past performance is important in determining its ability to successfully perform the services described in the PWS. The Government is interested in ensuring that the offeror has a successful history of recent and relevant past performance. To be considered relevant and recent, all references shall be similar in scope to the solicitation requirement, and services performed within the last three (3) years preceding the submission of this quote. d. A complete Contract Price/Cost Schedule attached to this solicitation, Attachment B - S02 RFQ Price/Cost Schedule 36C248-20-Q-1341. 2. FAR 52.212-3, Offerors Representations and Certifications Commercial Items Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. SECTION C - CONTRACT CLAUSES: Are attached to this announcement, Attachment A - S02 RFQ Clause and Provision 36C248-20-Q-1341. Additional instructions to Quoters. 1. Read the entire solicitation notice and attachments carefully. 2. Ensure you meet all the solicitation requirements and submit a complete quotation. 3. Quotes shall be submitted in English. 4. Ensure you are registered in the System for Award Management (SAM) and your registration is active. 5. Ensure you complete FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, online or submit a hard copy with your quote. 7. Due date for all question submission is September 12, 2020 close of business. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoters shall list exception(s) and rationale for the exception(s), if any.
