THIS IS A SOURCES SOUGHT NOTICE. The purpose of this RFI is to obtain market information and capabilities for planning purposes and to determine appropriate strategies to meet the Agency's requirements. This RFI is issued solely for information and planning purposes and does not constitute a Request for Proposal (RFP), Request for Quote (RFQ) or a commitment for an RFP/ RFQ in the future. In accordance with FAR 15.201(e), Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to the Sources Sought Notice. Information received from this Sources Sought Notice may be utilized in preparing potential solicitation documentation. It is the contractor's responsibility to identify proprietary information in response to this notice. The purpose of this announcement is to gain knowledge of potential qualified

sources, specifically SDVOSB s and VOSB s and their size classification. NAICS Code 238210 applies. However, small businesses are welcome also. The Phoenix Arizona VA Healthcare System (PVAHCS) is seeking potential sources capable of switching the ceiling mounted surgical light fixtures in operating room (OR) 1 with OR 2 so that OR 1 has a dual light fixture and OR 2 has a single light fixture. Potential contractors shall provide the capabilities to provide all services and products required as follows: A. The Contractor is a vendor approved and certified by the manufacturer, Skytron, to work on the Skytron light fixtures. B. The Contractor will cover travel and labor. C. The Contractor will come on-site to perform a pre-work evaluation to determine if any parts need to be ordered by the Agency for the installation. D. The Contractor will schedule his/her arrival with the Agency. E. The Contractor will check-in with the Agency before performing any work on-site. F. The Contractor will remove the single surgical light fixture from OR 1, Serial Number: 12113665, and install it in OR 2. G. The Contractor will remove the dual surgical light fixtures from OR 2, Serial Number: 12113662 and 12110667, and install them in OR 1. H. The Contractor will pull all electrical and data cables through the light fixtures conduit. I. The Contractor will wire the lights to the electrical/control panel in the OR. J. The Contractor will conduct a post-installation quality assurance test on the light fixtures in OR 1 and OR 2. The work will not be accepted if the lights are not fully functional. K. The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results. PVAHCS is located at 650 E.
