The County of Santa Clara (“County”) Technology Services and Solutions Department (TSS) is seeking responses from qualified firms and independent consultants to establish a qualified list of Proposer(s) who can provide skilled IT professional staff. This Request for Statement of Qualification (RFSQ) will identify Proposer(s) who have the qualifications, knowledge, and experience as noted in Appendix B – Healthcare IT Domain Position Response Form.
 
In a multi-year initiative, TSS is transforming and Consolidating IT services. The County is streamlining IT service offerings and aligning services to meet the business needs of County

agencies and departments.  Some of the IT services that will be streamlined and re-aligned include 
 
1. EPIC upgrades
2. EPIC module implementations
 
The Proposer(s) will have the capacity to meet all the qualifications, as listed under the Scope of Work (SOW) for each position title. The position titles have been grouped by IT Domain:
 
1. Business Intelligence, Analytics and Reporting
2. Project Management
3. EPIC/HealthLink
 
Proposer(s) are NOT required to provide responses for all three IT Domains. This RFSQ may result in a multiple award, preferably in each IT Domain. If an award is made, it is the County’s intent to award a contract with a term of up to five (5) years, unless terminated earlier or otherwise amended. 
 
From this qualified list of Proposer(s), the County will identify a selected number of Proposer(s) to award an agreement to provide services. This RFSQ is to provide the County with a qualification list. Once the County has determined the need, either for staff augmentation, assistance with a systems implementation or to provide a system or solution from conception to go-live, the Proposer(s) that fall under the specific IT Domain will be contacted. The Proposer(s) shall then respond to the request, along with an SOW and pricing. The County shall use this information, along with interviews (as needed) to determine the award for this request. The County reserves the right to remove Proposer(s) from the qualified list if Proposer(s) does not respond to County requests. The County reserves the right to contract with additional Proposer(s), using the qualified list, as needed.
 
The Qualified List of Proposer(s) may be maintained for up to five years. Being placed on the Qualified List does not guarantee the Proposer(s) a contract. The selection of Proposer(s) for contracts will be based on the needs and/or best value to the County.

Added on Nov 13, 2018:THE RFSQ HAS BEEN REPLACED IN ITS ENTIRETY. PLEASE NOTE THE FOLLOWING:
1. Please note that there has been no revisions to any of the submission documents.
2. A new Letter of Transmittal will need to be submitted, with an acknowledgement of the restatement.

THIS RESTATEMENT DOES NOT REQUIRE A RE-SUBMISSION. Vendors not wanting to resubmit must provide a letter stating that the previous submission under RFSQ-ISD-FY19-0079 and all references to RFSQ-ISD-FY19-0079 shall be updated and revised to read RFSQ-ISD-FY19-0079A.
