The Department of Veterans Affairs Medical Center, Salem, Virginia (VAMC Salem) is seeking potential sources to furnish all equipment, facility, professional and technical personnel services to provide routine primary care and mental health services for eligible veteran beneficiaries at a single site, Community Based Outpatient Clinic (CBOC) for a VA patient only clinic in Danville, Virginia.  The Contractor’s place of business must be within the city limits of Danville, VA., in order to allow patients to receive services in a timely, safe and efficient manner without compromising patient care.  The Contractor facility must meet applicable current JCAHO standards.   If the Contractor is JCAHO accredited, Contractor shall provide evidence of JCAHO compliance.  The Contractor shall have management plans for the seven (7) areas of the JCAHO Environment of Care Standards.  They are:  Security, Hazardous Materials, Emergency Preparedness, Life Safety, Medical Equipment, Utility Systems,
and Safety Management (Risk Assessment). The clinic must have either on-site phlebotomy and lab specimen preparation capability and radiology capability OR be able to demonstrate that a contractual relationship exists to provide off-site lab phlebotomy and specimen preparation and x-ray services to the clinic.  Clinic must be able to cover the following geographic locations:  counties of Henry, Patrick, Pittsylvania, cities of Martinsville, Danville, Hillsville, and Galax, Virginia and be located within the VA Medical Center, Salem, Virginia’s primary service area.  Contract services will be provided on a re-occurring basis and the contract Base Year period of performance will be from Date of Award through an initial 12 month period, subject to the availability of FY22 funding, with the option to extend the contract for (4) additional base year periods and five (5) additional Option years, for a total of 10 years. It is the VA’s intention to negotiate and award on a competitive basis, pending availability of funds.  The contracted personnel shall be board certified or board eligible and be located within the United States. Patients will be walk in patients Monday thru Friday, between the hours of 8:00 am and 4:30 pm at the contractor’s place of business. Cost basis/reimbursement will be by Current Procedural Terminology (CPT) codes on a per capitated rate.  See PWS for Estimated annual number of patients.
