for Emergency Preparations for Temporary Relocations, ITB-23-05-AD on or before Wednesday, November 23, 2022 @ 11:00 A.M. Local Time. Any bid received by the TOWN later than the above time shall remain unopened. Project scope includes but is not limited to: The Town of Fort Myers Beach seeks to contract with a qualified Contractor to do emergency preparations for temporary relocations. The scope is further defined and detailed within the Plans, Technical Specifications and General Conditions associated with this project. Contractor is responsible for reviewing all documentation associated with this project. All work performed shall follow all Federal, State, Local, OSHA and department mandated regulations and specifications for associated work. 1. PRE-BID CONFERENCE There will be a MANDATORY pre-bid meeting on Tuesday, November 15, 2022, at 11:00 A.M. Local Time, at the 2731 Oak Street Ft Myers Beach, FL 33931. The project will be built in strict compliance with the plans and
specifications in the Contract Documents. 2. DESCRIPTION OF WORK AND LOCATIONS • Remove and replace Fire alarm system to be returned to pre-hurricane status • Sprinkler system certified to code and returned to pre-hurricane status • Remove and replace All drop ceiling/tiles/grid • Remove and replace cabinets (kitchen, maintenance, and storage areas) to same quality as pre-hurricane status • Remove and replace all electrical to height of 8’ from ground level to return to pre-hurricane status • Remove and replace all wood and framing returning it to original on weight room around windows and doors • Remove and replace all interior/exterior doors to pre-hurricane status • Repair all soffits while adding 6” insulation above all • Replace fascia and soffits on lower portions of south and southeast corners of the structure • Remove and replace security system to pre-hurricane status. Contractor shall work with the Town’s contractor to coordinate with the existing system(239-241-1171) • Paint all interior walls 90” in height from floor to match existing colors. • Prep/paint all I-beam steel structural members around exterior to a height of 4’ above ground level to match existing colors. • Remove all tree and trash within 75’ perimeter of building exterior • Push back any debris piles to allow room for parking and add temporary fencing to separate debris piles from parking area • Remove and replace all bathroom partitions • Remove and replace lift station controls • Repair any damage to metal roofing preventing leaks • Remove and replace overhead roll up doors with hardware (2) 10’x12’ 4 • Mold/mildew remediation throughout the entire structure • All HVAC systems to be replaced • Replace sheetrock ceiling in women’s bathroom of main entry • Replace roll up window in kitchen area • Replace water heaters and water fountains in building • Remove and replace all diamond plate wainscoting in weight room area The CONTRACTOR agrees to perform all the work necessary to complete all items listed below. The OWNER has the right to accept or reject any or all part of a submitted bid. This project requires the Contractor to be licensed and insured in the State of Florida. The TOWN will require a Bid Bond and a Public Construction Bond on this project. The Town of Fort Myers Beach reserves the right, in its sole judgment in the best interest of the Town of Fort Myers Beach, to waive any informalities in any bids; to make award(s) including multiple awards; to waive any non-substantive, in Town’s sole judgment, irregularity or technicality in bids received, and/or to reject any or all bids. Local Time. Bids shall be opened at approximately 11:15 a.m. the same day, 2545 Estero Blvd., Fort Myers Beach, FL 33931. Complete sets of Bid Documents must be used in preparing the Bid. The TOWN will not assume any responsibility for errors or misinterpretations resulting from the use of incomplete sets of Bid Documents. 1. SUBMITTAL OF BIDS 3.1 BID SUBMITTALS MUST COMPLY WITH THE FOLLOWING TO BE VALID: 3.1.1 Bid must be received by the TOWN via DemandStar.com, prior to bid closing time. 3.1.2 Completed Official Bid Form (#FMB:003), bid must be properly signed and, where applicable, corporate and/or notary seals attached. The complete, computerized bid schedule must be submitted with the bid at bid submission as applicable. 3.2 EACH FIRM MUST SUBMIT VIA DemandStar.com prior NOVEMBER 23, 2022, 11:00 A.M. LOCAL TIME: FAILURE TO COMPLY AND SUBMIT MAY RESULT IN DISQUALIFICATION OF THE BID. 3.2.1. Signed and notarized Affidavit of Certification Immigration Laws (#FMB:100). 3.2.2. Contractor ‘s Qualification Questionnaire (Form #FMB:004) Note: A current copy of Form #FMB:004 can be placed on file with the TOWN and must be kept current every 12 months. 3.2.3. Non-Collusion (#FMB:021) 3.2.4. Materials List (#FMB:023) 3.2.5. Sub-Contractors List (#FMB:024) 3.2.6. Signed Acknowledgment of Receipt of Addenda (if any). FAILURE TO SUBMIT ANY OF THE FOREGOING MAY RESULT IN THE BID BEING FOUND NON-RESPONSIVE. 3.3 THE FOLLOWING MUST BE SUBMITTED WITHIN TEN (10) CALENDAR DAYS AFTER RECEIPT OF NOTICE TO AWARD; FAILURE TO SUBMIT MAY BE GROUNDS TO RESCIND AWARD: 3.3.1. DEFINITIONS 1.1. “ADDENDUM” means any additional Contract provisions, in writing signed and sealed by the CONSULTANT, if applicable, issued by the TOWN prior to the receipt of Bids, which clarify, correct, change or interpret the Bidding Documents or the Contract Documents. 1.2. “ALTERNATE BID” means an amount stated In the Bid to be added to or deducted from the amount of the Base Bid if the corresponding change in project scope or materials or methods of construction described in the Bidding Documents is accepted. 1.3. “BASE BID” is the sum stated in the Bid for which the Bidder offers to perform the Work described as the base, to which Work may be added or deducted for sums stated in Alternate Bid. 1.4. “BIDDER” is any individual, firm, partnership, joint venture, or corporation submitting a bid for this project, acting directly or through an authorized representative. 1.5. “BID” is a complete and properly signed proposal to do the Work or designated portion thereof for the sums stipulated therein, submitted in accordance with the Bidding Documents. 1.6. “BID BOND” is a security in the form and amount required by the TOWN pledging that the BIDDER will enter into a Contract with the TOWN on the terms stated in the Bid. 1.7. “BIDDING OR BID DOCUMENTS” are the Request for Bids, the Notice to Bidders, Instructions to Bidders, sample forms, Addenda issued prior to receipt of Bids, the Bid Proposal Form and the proposed Contract. 1.8. “CONTRACT PRICE” means the total monies payable to the CONTRACTOR under the Contract Documents. 1.9.
