Description
Scope of Services City of Toledo (City) is requesting a vendor to provide prescription drug benefit management services for the City’s Health Insurance Plan (Plan). The City desires to contract with a qualified, experienced Pharmacy Benefit Manager (PBM) capable of providing prescription drug benefit management services to a large, self-insured health plan, and having prior experience directly related to the services requested in this RFP. The initial term shall be for 36 months with options to renew for two (2) additional one-year terms. Please explain your three-year strategy when submitting a proposal.
Other Details Submission Proposals for this project can only be accepted via e-bid though the City of Toledo’s PlanetBids portal. Bids prices shall be to two (2) decimal places (i.e. $0.00). In order to be responsive, all proposals must have the following with submittal: Project Paperwork* (Acknowledgment and Acceptance of Conditions Contract Specifications; EEO Form; MBE Good Faith and Goal Commitment; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; Primary and Subcontractor Certificates and Debarment and Suspension Certificate), signed contract by the vendor* child support affidavit*, bureau of worker’s compensation certificate, certificate of insurance, naming the City of Toledo additionally insured, Re-Pricing Exercise,* and Fee Schedule.* Please note the above paperwork due with the proposal are all separate mandatory attachments on your e-bid separate from proposal. *Located on the “Documents” tab of this bid Bidders are encouraged to start their e-bid prior to the bid due date, as you have the capability of saving and re-entering your bid submission.
