Bids to Owner's Agent Description Scope of Services The County of Stanislaus (County) is issuing this Request for Proposals (RFP) seeking a qualified and experienced non-profit to provide services which enable older adults to continue to age in place within their home by providing rent funding assistance to vulnerable, low-income older adults to stabilize their housing; provide a program, at no-cost to the recipient, of services to match people to share suitable housing options; and assistance to help low-income older adults to actively engage in identifying long-term solutions to their housing instability for the Aging and Veterans Services, Area Agency on Aging, located at 3500 Coffee Road, Suite 19, Modesto, CA 95355. Other Details History The Stanislaus County Department Area Agency on Aging (AAA), in conjunction with the Commission on Aging, serves as the AAA for Stanislaus County. The Commission on Aging is the AAA’s advisory council, and the Stanislaus County Board of Supervisors is its governing body. As such, the AAA receives federal and state funds under the authority of the Older Americans Act of 1965, as reauthorized in 2006. The AAA awards contracts to provide direct services to residents of Stanislaus County who are sixty (60) years of age or older, in this instance to provide aging in place services for older adults. The Federal Older Americans Act, enacted by Congress in 1965 to promote and aid in the development of programs to help older adults aged sixty (60) and over remain at home for as long as possible, promote the rights of older individuals, and advocate for individuals who live in long-term care facilities. Funding for such programs is distributed through State Units and Area Agencies on Aging. Title 111-B of the Older Americans Act provides funding for these supportive services. https://aging.ca.gov/download.ashx?lE0rcNUV0zac5x5tGZUebw%3D%3D Program Overview Aging in Place is a new concept that can result in less injury, retaining older adults in their homes, and offers a significant cost savings to health insurers, families, and public agencies. The target population is adults aged sixty (60) and older who are not receiving institutionalized care and are instead aging in their home with support and supplemental services to remain in their homes and communities as they age, rather than relocating or moving into an institutional setting. For the cash/material aid program, the provider (“Contractor”) will arrange for and provide assistance to participants in the form of direct payments to their landlord/property management company for a period of two (2) years or less. The provider (“Contractor”) will verify the eligibility of the participants as specified in the Scope of Work as outlined in Exhibit A of the Agreement attached. For the HomeShare program, the provider (“Contractor”) will match people who have a room to rent with people seeking housing; low-income older adults receive priority. As part of the program, assistance will be given to low-income older adults to identify long-term solutions to their housing instability with one-on-one personalized HomeShare consultations.
