Custom Made Foot Orthotic

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation; quote is being requested and a written solicitation will not be issued.The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Fort Shafter, HI 96858, intends to solicit, negotiate, and award a Blanket Purchase Agreement (BPA) for Custom Made Foot Orthotics for the 121st CSH/BAACH. This is a Request for Quotation (RFQ).  This solicitation number is W81K02-18-T-0069, which will be solicited as a sole source.  The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95.  The Standard Industrial Classification Code is 6515, size standard is 1,000 employees and the NAICS code of 339112. Provisions at 52.212-1, Instructions to Offerors - Commercial, and its addendum

applies to this acquisition and a statement regarding any addenda to the provision.  See Attachment 1. Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, and its addendum applies to this acquisition.  See Attachment 1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executives Orders - Commercial Items, applies to this acquisition.  See Attachment 1 for FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Blanket Purchase Agreement. Contract Requirements - The successful offeror shall be required to provide Custom Made Foot Orthotics listed in Attachment 1. Point of contact for this solicitation is Mr. Tae Kim; (808) 438-5131; email: tae.h.kim76.civ@mail.mil. Response must be received by April 25, 2018 @ 10:00AM HST.
