The Department of Veterans Affairs, Networking Contracting Office (NCO 01) located in Providence, RI intends to enter into a sole source procurement, pursuant to the authority of FAR 6.302-1, with Alliant Enterprises, LLC, for the following requirement: STATEMENT OF WORK (SOW) UVC Robot Install and Services 1. Background. The contract is required to ensure that the Providence VA Medical Center is providing the best disinfection to the Providence Medical Center. 2. Scope. The Veterans Affairs, VA Medical Center, Providence, intends to solicit and award a contract to provide installation and preventative maintenance service for the UVC Robot System Model Number 2280 located in the Providence Medical Center facility of the VA Medical Center, located at 830 Chalkstone Avenue. Services shall be in accordance with the terms, conditions, provisions and schedule of the contract. Specific Tasks-Deliverable Product. 3.1.1 Contractor shall provide all labor, materials, equipment, transportation,
lodging and supervision necessary to perform preventative maintenance for the UVC Robot system Model Number 2280 located in the Providence Medical Center facility at the 830 Chalkstone Avenue. Service Performed Under Maintenance Plan Units Description Frequency 1 Initial Equipment Setup Initial Setup 1 Shift User Training Initial Setup/As Needed 1 Management Training on the Documentation System Initial Setup/As Needed 12 Process Review and Workflow Solutions Monthly 12 Room barcode setup for usage documentation Initial Setup, Monthly 12 Steri-Trak setup and website access Initial Setup, Monthly 12 Steri-Trak Replication Monthly 12 Phone Support Monthly 12 Non-disposable parts As Needed, Monthly 12 Lamps As Needed, Monthly 12 Lamp Recycling As Needed, Monthly 1 Scheduled Preventive Maintenance per year Annually 12 Service Calls as many required to maintain uptime guarantee Monthly 12 Bulb Replacement As Needed, Monthly 12 Technical Support Services Monthly 3.3 Hours of Operation All work for this contract is to be done during normal business hours. 4. Performance Monitoring Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract. 4.1 Direct Observation-(Can be performed periodically or through 100% surveillance). 4.2 Periodic Inspection-(Evaluates outcomes on a periodic basis. Inspections may be scheduled (Daily, Weekly, Monthly, Quarterly, or annually) or unscheduled, as required. a. ACCEPTABLE PERFORMANCE The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action. b. UNACCEPTABLE PERFORMANCE When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file. When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's program manager. The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability. Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the Contracting Officer (CO). 5. Security Requirements Vendor will not have access to VA Sensitive Information, patient records, or data. For repairs or services performed during normal work hours, 7:00 a.m. to 4:00 p.m. at the Providence VA Medical Center, contractor s repairman shall report to the Security Office located at the entrance of the Medical Center. Contractors shall sign-in and obtain a badge and return the badge at the end of the day. After all work is complete, the contractor s repairman shall again report in person to the room listed above, submit a written service report outlining the services performed and completed to include location and parts used. For services performed outside normal working hours, the contractor s repairman should report to the Security Office at Providence VA Medical Center. After all work is complete the contractor s repairman shall again report to the Security Office where the repairman shall submit the report required above. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Solicitation to provide all labor, material, equipment, transportation, lodging and supervision necessary to perform installation and preventative maintenance for the UVC Robot System, Model Number 2280 located at the Providence VA Medical Center at 830 Chalkstone Avenue. The Providence VA Medical Center will purchasae necessary parts for the UVC Robot System during the performance of this contract. The Providence VA Medical Center will not furnish the test equipment. It is the responsibility of the contractor to bring the appropriate test equipment; and/or supplies necessary for the test equipment to complete the work as required within. 7. Other Pertinent Information or Special Considerations. All work shall be performed by personnel qualified to work on the specific equipment. All work performed shall compliant with AAMI and CMS guidelines and shall be designed to assure that the Providence VA Medical Center is providing the best possible water to the patients. 8. Place of Performance. The Department of Veterans Affairs Medical Center Providence, 830 Chalkstone Avenue, Providence, Rhode Island 02908. The UVC Robot System Model Number 2280 is located in the Medical Center. 9. Period of Performance This contract will be for a base + four (4) option years. The anticipated start date is March 1, 2023. The NAICS code for this requirement is 811310. Installation and Services for the UVC Disinfection Robot with Smartdosage UV & Steri-Trak Digital Documentation (UVC Robot System, Model Number 2280). Alliant Enterprises, LLC is the only vendor that is capable of satisfying this requirement because Alliant Enterprises is the exclusive provider of Skytron products. This notice of intent is not a solicitation nor is it a request for proposal. However, all responsible sources may submit a capability statement demonstrating the ability to provide the requested services, in writing by Friday, February 3 at 12PM EST time. Information submitted in response to this notice will be used solely to determine whether competitive procedures could be used for this acquisition. A determination not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
