The Department of Veterans Affairs, VISN 2, Network Contracting Office (NCO2) is currently conducting a market survey for firms capable of providing the following: MEDICAL WASTE (SHARPS) DISPOSAL The contractor shall furnish all supplies and services to operate a sharp waste container distribution, collection and disposal system in accordance with all Federal, state, and local regulations. 2. The Contractor must provide a license and permit as a regulated medical waste/sharps hauler for the removal of Department of Veterans Affairs waste and is responsible for the removal cost. Contractor shall submit copies of license/permit with its offer. 3. The Contractor shall provide a one day per week full service, turn-key systems including the following: Technician(s) must deliver carts on the day of service (no prior exchange permitted). Technician(s) must
collect utilized sharps containers and replace with clean sharps containers from all sharps container locations sites within main Building 200 and 200N, outlying buildings (6, 8, 11, 61, 62, 63, 64, 65 and 92) and satellite primary care clinics. Technician(s) must collect utilized sharps containers and replace with clean sharps containers from all locations. Technician(s) will perform the necessary collection/packing/storing of used sharps containers for collection/transport by a licensed regulated medical waste hauler. The Contractor must transport the waste off the premises at the completion of the container exchange, no storage permitted and subcontracting transportation of waste not permitted. 4. All contractor personnel entering into any/all New York campus facility locations and performing any duties or services stated within this section must be current employees of the Contractor or its subcontractor, must obtain VA-issued identification and shall be trained in the collection/packaging/transporting of regulated medical waste and sharps in accordance with all federal, state, and local laws. 5. Contractor-assigned personnel must report to Safety Office prior to commencement of any/all work assignments. A log is provided by Safety Office to record service visits as well as list concerns and problem calls. 6. Contractor-assigned personnel must view and annotate in the log before service is performed and after service has been performed. Concerns and/or problem calls must be addressed immediately at the commencement of the service visit and recorded in the log as completed . 7. The Contractor shall provide approximately 525 rigid, heavy duty, puncture-resistant, leak proof, vertical/direct drop reusable and/or disposable, generally heavy duty plastic containers with closable cover for syringes, needles, scalpel blades, and glassware used in the diagnosis of patients of the following sizes: Small 1 gallon-80 units Large 3 gallon-300 units Large 8 gallon-145 units 8. All containers must be delivered to the Medical Center clean and disinfected after each use. Where containers are fastened to the wall, contractor must provide a lockable uniform outer cabinet. Cabinet must be capable of accepting either the 2 gallon or 3 gallon container. 9. All containers and/or outer cabinets will be clearly labeled with a biohazard symbol and have a visual display window or means to check the level of the container or inner container. 10. Where applicable, cabinets shall have a sloped top to prevent placement of sharps or any other items on top of unit instead of into unit. 11. The Contractor is responsible to setup a bar code system, providing Safety Office with information regarding improperly disposed of non-sharps in the containers and weights/frequency of container replacements by room number. 12. The Contractor must survey each facility annually and maintain a location log/inventory for subsequent employees of Contractor if service is required for/by various technicians. Performance Work Statement 13. The Contractor must check, collect and replace containers at all container sites outlined in the survey. Collection services will be once per week. 14. The Contractor must replace all containers that are more than approximately one-third (1/3) full, to ensure no overfills between service days. 15. The Contractor must replace any/all containers which have not been filled and/or serviced in over 30 calendar days, regardless of its content or volume. 16. The Contractor should have service infrastructure to handle all applicable issues related to sharps waste disposal and service. 17. The Contractor should be able to complete all terms and conditions of the contract immediately following the award of the contract. There are to be no delays in delivering the service and/or product. 18. All containers, boxes and/or transports packaged and/or filled for transport must be labeled (prior to loading for transport) as follows: Department of Veterans Affairs Northport VA Medical Center 79 Middleville Road Northport, NY 11768 Department of Veterans Affairs Riverhead CBOC 300 Center Drive, 2nd Floor Riverhead, NY 11901 Department of Veterans Affairs East Patchogue CBOC 4 Phyllis Drive Patchogue, NY 11772 Department of Veterans Affairs Bay Shore CBOC 132 East Main Street Bay Shore, NY 11706 Department of Veterans Affairs East Meadow CBOC Nassau University Medical Center 2201 Hempstead Turnpike, Building Q, 1st Floor East Meadow, NY 11554 Department of Veterans Affairs Valley Stream CBOC 99 Central Avenue Valley Stream, NY 11580 19. The Contractor shall provide the COR with a duplicate copy of all address labels to be applied to items in item number 18 above. 20. The Contractor shall provide copies of all medical waste transporter permits and licenses for regulated medical waste handling. A medical waste transporter's permit (NYSDEC part 364) is required. Final disposal of sharps waste will be in an authorized disposal facility. 21. No regulated medical waste is permitted to leave the medical center until a manifest is properly completed and signed by an authorized VA employee. 22. The Contractor is responsible for completing the Regulated Medical Waste Tracking Form prior to leaving the medical center. 23. The Contractor is responsible for ensuring Copy 1 (Generator Copy) of the Regulated Medical Waste Tracking Form is received by the Northport VAMC within 35 days of the date of waste acceptance by the Contractor. 24. The Contractor shall provide clean replacement containers as needed on or before scheduled service days. 25. The Contractor shall provide on-call service as needed for emergency service directly associated with sharps disposal and/or sharps container/service. 26. The Contractor shall provide sharps removal service to all medical center areas to include medical wards, clinics, exam rooms, restrooms, crash/medication carts and various areas throughout the medical center. 27. Sharps service can be performed on weekdays (Monday through Friday) 8:00am to 4:00pm. The Contractor shall provide their own storage unit/structure for storage of new clean containers as well as containers used and ready for disposal. Location must be approved by the VA prior to placing on station. 28. The disposal company used by Contractor shall recycle (if applicable) all sharp containers in compliance with Federal, State and/or Local recycle efforts. 29. The Contractor is required to recycle all appropriate plastics and provide data to COR. All trash produced by the Contractor (boxes, bags, packing materials, etc.) will be collected and removed by the Contractor to a location designated by the COR. 30. Contractor shall arrive on collection days with a fully permitted vehicle for use in collecting and transporting waste from medical center and outlying buildings. 31. All disposal and transportation of sharps medical waste shall be accomplished in accordance with appropriate Federal, State and local regulations and with the most current edition of OSHA requirements. 32. Monthly Sharps report: The Contractor is required to provide a monthly sharps report with collection information that includes, but is not limited to outlining the quantity of containers; sharps disposal weight; total plastic diverted. These reports shall be generated by the Contractor and provided by the tenth day of each month. 33. Contractor must have demonstrated experience servicing similar hospitals with reusable containers for an extended period of time. Contractor must have the infrastructure to handle all applicable issues related to sharps disposal service. 34. Contractor shall provide evidence of insurability. The minimum limits of the insurance policy must be $1 million dollars for the transport of regulated medical waste. 35. Contractor should be able to comply with terms and conditions of the contract. No delays in service should be incurred by the Medical Center.
