Service report of the repairs shall be a detailed documentation of the repair. Engineering point of contact is Judy Sumner, 334-272-4670 extension 1-4432. Point of contact is to be contacted for any information concerning this work.PURPOSE: PROVIDE SERVICE, TO INCLUDE PARTS, LABOR AND TRAVEL, for repair of Carl Zeiss Field Analyzer, HFAI, s/n HFAI-13642, EE67793 to manufacturer s specifications using OEM parts and repairs performed by OEM factory trained technicians.EE# S/N: Location: Fault/Fault Code67793 HFAI-13642 Tuskegee Optometry REPAIR AND CALIBRATIONWARRANTY: Contractor shall provide warranty on the integrity of parts and service on the Carl Zeiss Field Analyzer, HFAI, s/n HFAI-13642, EE67793, location 221-003-TU, Tuskegee Optometry Clinic. The warranty period shall be for 90 days after completion of work.SCOPE OF WORK:Repair Carl Zeiss Field Analyzer, s/n HFAI-13642 to manufacturer s specifications using OEM parts.Repairs performed by OEM factory trained technicians.Contractor provides all parts, calibration, labor, misc., and shipping (to & from).Contractor refurbishes, repairs and make completely operational the Carl Zeiss Field Analyzer, HFAI-13642. Contractor uses certified OEM trained technicians and uses OEM parts for the repairs,Contractor performs repairs to manufacturer s specifications,Contractor provides detailed and separate documentation of the repair/refurbishment.Contractor performs repairs within 2 days from date of receiving purchase order.NOTES: All modifications shall be reviewed and approved prior to execution.LOCATION: 2400 Hospital Rd, Tuskegee, AL
