:04/26/2024 2:00 PM LEGAL NOTICE TOWN OF FARMINGTON, CONNECTICUT REQUEST FOR PROPOSALS MEDICAL SERVICES INCLUDING PRE-EMPLOYMENT EXAMS, WORKERS COMPENSATION AND PRIMARY CARE PROVIDER SERVICES The Town of Farmington, Connecticut is requesting proposals from qualified firms to provide pre-employment physical examinations and workers compensation primary care provider services as well as other medical services as more fully described in the following RFP. Proposals will be received by Kathryn Krajewski, Assistant Town Manager, until 2:00 p.m. on Friday, April 26, 2024, After this time, no further proposals will be accepted. Request for Proposal forms may be obtained in the Town Manager’s office located at Town Hall, 1 Monteith Drive, Farmington, CT 06032 from 8:30 a.m. until 4:30 p.m. Monday through Friday except holidays.
