1.01Purpose
 
The purpose of this Application Guidance is to provide eligible applicants
information on completion of the application for the Childhood Lead Poisoning Prevention Program (CLPPP) Services covered by this application include the coordination of CLPPP activities within their defined jurisdiction.  At a minimum, program activities include medical case management, environmental case management, data management, public education and outreach, and training.
 
1.02Project and Contract Period
 
Funding during the defined project period is dependent on approval of the Application, contractor performance during the fiscal year, compliance with general and special conditions of the contract, availability of project funds, program modifications, or any other grounds determined by the Department to be in the Department’s best interest. The Department expects the contract period to be a one-year term from July 1, 2018 to June 30, 2019.
 
The issuance of this Application Guidance in no way constitutes a commitment by the Department to award a contract.
 
1.03 Eligible Applicants
 
Applicants must meet each of the following eligibility requirements for consideration.

Due to limited funding and changes in how the childhood lead program is administered locally, only current Local Boards of Health (LBOH) contracted with the program will be the eligible to apply for and serve as the contractor for the July 1, 2018 - June 30, 2019 (FY19)contract period. The LBOH listed below have jurisdiction over public health matters within their defined geographic areas for childhood lead poisonings. A Local Public Health Agency may not apply in response to this posting.

1.04Service Delivery Area
 
Service Delivery area is the county or counties represented by the eligible applicant.
 
Refer to the FY18 Childhood Lead Poisoning Prevention Program Contactors & Service Areas Table posted under the Attachments portion of this Funding Opportunity posting for service counties for each eligible applicant. Any service delivery area changes for this application process must be agreed to by existing contractors and applicable boards of health, and approved by the Department. Submit requests to Kevin Officer at kevin.officer@idph.iowa.gov.  Written approval must be obtained prior to submitting an application. 
 
1.05Funding
 
The source of funding is state appropriated funds.  Funding percentages for each CLPPP program service area is based on a risk model that uses county level data to determine the following variables: (1) percent of pre-1950 housing, (2) percent poverty, (3) population under 6 years in age, (4) percent of children tested, (5) percent of tests 10µg/dL or greater, and (6) percent of confirmed tests 20µg/dL or above. These variables establish the performance and burden portion of the risk model.  Each variable is equally weighted and totaled to provide a percent of funding for each of the defined CLPPP program service areas.  The Projected Funding Allocation Table is posted under the Attachments portion of this Funding Opportunity posting.  Applicants may apply for the allocated amount shown in the table. 
 
Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Department’s best interests.

I can find our dollar amount in the Projected Funding Allocation Table (not marked table 1), but no population data. Do you want us to just input what the US Census states is our county population data or should there be another table to reference?An Iowa County population table has been placed in the funding opportunity page for your reference.06/01/2018Per the RFP document (page 20) and the review tool document we should be entering information regarding subcontract plans. I do not see a subcontract form for us to complete within our grant application.In lieu of submitting a subcontract form approved applicants will upload copies of subcontracts to their FY19 grant site in the Correspondence component of IowaGrants.gov.06/06/2018Where are the numbers for the 2015-2017 baseline? IDPH tracking portal? I think I’ll need to know that info to see how much we need to improve. I know 3 year old birth cohort leaves room for improvement. 6 yr. old cohort is looking pretty good.County level childhood lead data can be obtained from the Iowa Public Health Tracking Portal website at https://tracking.idph.iowa.gov/Health/Lead-Poisoning.06/06/2018What happens if we don’t meet a deliverable? What is the penalty by quarter or year end?There is no penalty for not meeting a deliverable. By linking deliverables to performance measures provides the department with information on how well the CLPPP program is meeting key program objectives. Also, the contractor will receive fixed cost payments for the
corresponding period even if there is no eligible population for the stated deliverable requirement.06/06/2018How will the “ensure” completion of AEA, nutrition & medical going to be verified?The department anticipates that once a referral is made for a developmental assessment, nutritional evaluation, or medical examination the CLPPP contractor will follow up to confirm or verify the service was provided and record the activity in HHLPSS.06/06/2018How do we “ensure” Title V, WIC, Empowerment will have the kids lead tested? Are you asking all to draw blood or are you asking all to promote lead testing? Or are you asking us to educate/outreach to these entities so they will in turn give the message to their participants?As the coordinator of CLPPP activities and services within your defined service area the department anticipates the contractor will reach out to all child health care entities to educate them on Iowa’s blood lead testing and follow-up testing guidelines, especially those entities that conduct blood lead screenings or testing.06/06/2018EBL inspections becoming or contracting with a Medicaid certified lead inspector – does that include IDPH? I thought there was going to be a visual inspection option. Is that covered by Medicaid? I've only had 9 children that qualified for inspection in the last 2 year; two were siblings, one moved, one was a visual completed by a nurse, one had been inspected years ago & one no one could ever get a hold of the family.IDPH is not a Medicaid provider and therefore has no plans to conduct EBL inspections in homes of children that are Medicaid eligible. During the FY19 contract period EBL inspections must be completed in accordance with Iowa Administrative Code Chapter 70 and Iowa Childhood Lead Poisoning Prevention Program Case Management Protocols, which currently do not allow for the use of a visual risk assessment.06/06/2018Tell me more about a plan needed when the grant money no longer exists. This seems to be implying FY19 is the last year for this type of funding. Is there an end date to this particular funding?As long as state law requires the implementation of a grant program and state appropriated funds are allocated to the department it will continue to provide funding for a childhood lead poisoning prevention program.06/06/2018Where is the list of certified lead inspectors?The current list of Iowa certified lead inspectors/risk assessors will be posted in the Attachments section of the application.06/14/2018If we are not currently enrolled as a Medicare/Medicaid provider, are we still eligible to apply for this grant?

Thank you.Yes. The Medicare/Medicaid provider requirement allows CLPPP contractors to seek reimbursement for childhood lead services that are covered by medicaid for eligible children with elevated blood lead levels. If the applicant is not a Medicare/Medicaid provider then an explanation needs to be provided on how medicare/medicaid services will be reimbursed without using CLPPP funds.06/14/2018What are the qualifications for the HHLPSS Data Manager? Who would be responsible for this position?

Thank you.There are no specific job qualifications, standards, or requirements for the HHLPSS Data Manager. This person can be a data entry staff person, a case manager, clinical nurse, or a team of staff members with one designated as the lead person. The HHLPSS Data Manager will be the primary person the department contacts regarding missing information in a child record or address record.06/15/2018When will the baseline data for deliverables be available to eligible applicants to complete the application workplan?For the application process the applicant doesn’t need to know current baseline measurement data for each deliverable. The applicant should describe how they plan to coordinate services or activities in each service area county to meet the specific deliverable according to childhood lead program protocols.06/15/2018Are there funds funds to cover any blood leads for children under six years with no insurance/Medicaid coverage? How will those be accessed?Historically the department has allowed CLPPP contractors to utilize grant funds to cover blood lead testing and blood lead analysis for uninsured and Medicaid ineligible children. This allowance continues under this funding opportunity.06/15/2018In the Environmental Measures for FY19 it states (C13.) Follow up on 100% of investigations completed in previous years at least every 12 months until lead hazard remediation is completed? Is this a reversal of the change a few years ago? The FY18 Contract states "If lead hazard remediations still exist after 10 years and no child less than 6 years live in the dwelling, and the dwelling is not a child-occupied facility, the Contractor may discontinue follow-up activities."No, this is not a reversal. Assume “previous years” to mean last 10 years from address being open. We will update this language in the FY19 contract to reflect FY18 language and language in the CLPPP protocol document.06/19/2018Where can an applicant upload the Multiple Board Sign-In Sheet for counties in the applicant's service area?Submit Multiple Board Sign-Up Sheets to the Program Contract Manager. Email completed sheets to Kevin Officer at kevin.officer@idph.iowa.gov. A copy of the Multiple Board Sign-Up Sheet will be uploaded to the applicants FY19 grant site in Iowagrants.gov.06/25/2018Do all children covered by Medicaid need to be tested according to the high-risk schedule?No. Use the risk questionnaire to determine if a child needs to be tested according to the low- or high- risk testing schedule, especially if it's the child’s initial blood lead test.06/26/2018With the new claim reporting structure described in the RFA guidance, will payment for case management services be withheld when a family does not comply with staff?

For example, in a given quarter nine out of ten (90%) may follow through with staff, which less than the 95% required by the grant performance measures (B3, B6-B8, B10, and C11). Of similar concern is the reimbursement for B4 and B5 performance measures which are highly dependent on family participation, and require 75% compliance. If one case does not comply with our agency, is the case management work that went into the first nine cases not reimbursable when that target percentage is not achieved?Refer to Amendment #2 and Revised Contract in the FY19 CLPPP funding opportunity notice for further guidance.

No, payment will not be withheld if a family does not comply with a staff recommended follow-up test (for example) or if the contractor does not meet a performance measure for some other reason. As stated in an earlier question, there is no penalty (i.e., loss of funding) for not meeting a performance measure associated with a specific deliverable.

Performance measures are in place to provide the contractor with information on how well their program is performing in meeting specific program objectives. It also provides the Department with information on determining future program needs as far as education and outreach, funding, and resources.06/26/2018Similar to the question about reimbursement for case management work when a quarterly performance measure falls below the target percentage, how will performance measure tracking and claims be handled when management of cases crosses two quarters?

For example: Outreach and services begin in the second quarter for a case, and are completed in the third quarter.As stated earlier, funding will not be withheld if a performance
measure is not achieved for a given quarter. In the Quarterly
Progress Report the contractor will be required to provide information on
why a performance measure was not met for the quarter.

section of the Funding Opportunities notification for the Childhood Lead
Poisoning Prevention FY19 application in Iowagrants.gov.07/06/2018In a previously answered question, it is stated that there is no penalty for not meeting a deliverable. However, the contract also states and the webinar explained that reimbursement will be based on successful performance in meeting the deliverables. So are we to assume that there is no payment for a deliverable that is not met in any particular quarter, and no means to recoup that money during the contract? Many of these deliverables are holding the contract-holders accountable for activities that we do not directly complete - we must rely on the parents to bring the children in for testing, and the providers to do the proper testing and we are being reimbursed based on their performance, not our own. 95% is a pretty tall order - so there will be no reimbursement if 15 out of 16 children receive proper retesting (only 94%), but a contract that has no eligible population for the quarter and does nothing gets full reimbursement? This doesn't seem right - can you please clarify?Refer to Amendment #2 and Revised Contract in the FY19 CLPPP funding opportunity notice for further guidance.

The Department understands there are circumstances that occur in
delivering services or trying to get a family or property owner to comply.
The Department has no intent in withholding funding from contractors for
not meeting a quarterly performance requirement. The Department
expects contractors to make every effort to work towards meeting or
exceeding a performance measure and document their efforts in the
quarterly progress reports.

Quarterly progress and case management reports, along with other
support documentation, will also be used by the Department in
determining if the contractor has made every effort to meet a performance
measure.07/06/2018Is there no longer going to be any reimbursement for clearing a property? Are we being asked to conduct clearance inspections? There is certainly a cost to us in the time spent doing these inspections.Under the FY19 contract the Department will no longer make
reimbursements based on direct services provided, but on the results of
services or activities occurring within the contractors jurisdiction. Once
quarterly funds are disbursed by the Department to the contractor the
funds can be used to pay for any childhood lead poisoning prevention
program activity or service, including reimbursement for clearance
inspections.07/06/2018Is there going to be a database or means to track these deliverables? HHLPSS is pretty limited on the data that it allows us to track and the reports that we are able to generate. Will we each be expected to develop some type of tracking mechanism or will we receive training on how to generate the data that we need?The Department is currently developing a system for tracking
performance data, similar to childhood lead data currently reported on the
Iowa Public Health Tracking Portal. At the end of each quarter the
Department will provide each contractor with a performance data report.07/06/2018The only source that we have found for a Nutrition Evaluation is the WIC nutritionist. We do have an MOU with our WIC that they will provide nutrition evaluations to lead poisoned children who qualify for WIC. If the child does not qualify for WIC, the fee is $25 per hour. Sometimes we have been unable to convince the child's guardian to make an appointment with WIC and especially if they have to pay for the service. For the family where this is the case, is the CLPPP Case Manager allowed to complete a nutrition evaluation for the family and enter a completed event in HHLPSS?Yes, a certified nurse or medical provider can provide nutritional
information to the family during an office/clinic visit or home nursing visit.
The CLPPP Case Manager or Data Manager would be responsible for
entering the proper information into HHLPSS in order for the contractor to
receive credit for meeting a performance requirement.07/06/2018On the application form on Section D, Education & Outreach Measures, it states to give presentations about childhood lead poisoning prevention to each of the specified groups at least once during the contract period. Does this mean just formal presentations with power point and audience or is it acceptable to achieve the deliverables using social media, participation in community events serving target population where it is an open forum to educate one on one with parents? This seems confusing with the description under required activities section E. Education/Outreach page 14.Public presentations to groups through formal presentations as a
speaker, as a vendor at a community event, or at any other public meeting
or gathering where face-to-face conversations about childhood lead
poisoning prevention can occur is acceptable. Social media can be used
as long as some type of metric is used to measure audience reached and
click-throughs to get additional information on childhood lead poisoning
prevention.07/06/2018Can you provide clarification on your definition of a “thorough physical examination”? What specific documentation would you expect to see in the child record?At a minimum, the Department would expect testing for iron
deficiency and an abdominal x-ray for lead paint chips for child with an
elevated blood lead level requiring a home inspection.
