The Work generally includes, but is not limited to: • Supply and
installation of approximately 40m of 300mm diameter PVC sanitary sewer pipe inside a 610mm OD steel casing pipe using guided horizontal auger boring method • Supply and installation of approximately 40m of 675mm diameter PVC storm sewer pipe inside a 1067mm OD steel casing pipe using guided horizontal auger boring method • Traffic control throughout construction involving coordination and traffic management plan approval from the City • Settlement and vibration monitoring throughout construction • Design and implementation of dewatering of exit pit and/or entry pit, as required • Site restoration, road paving, clean-up, and landscaping work • Other related works and/or matters including but not limited to testing of installed pipe, silt and erosion control, tree/shrub removal and replanting, adherence to Japanese beetle regulation, coordination with the public, residents, City of Vancouver, BC Hydro, Telus, FortisBC, VPD, Fire Departments, BC Ambulance, First Nations, various underground utility undertakers, and other construction interfacing works and contractors, compliance with safety and health requirements, quality control, and environmental protection, etc. Please see the Buyer Attachments section of this website to access the Tender Documents. Group 4.1: List of Subcontractors Instructions: The Tenderer should list below all subcontractors it intends to use in the performance of the Work and provide the requested information for each subcontractor. The City supports supplier diversity and seeks to create economic opportunities for businesses that are majority owned, controlled or certified by under-represented populations. Accordingly, the Tenderer should indicate below whether any of the proposed subcontractors meet any of the applicable criteria. 4.1.1 Submit list of Subcontractors using the attached template Group 5.1: List of Suppliers Instructions: The Tenderer should submit a list of any suppliers and manufacturers it intends to use to supply specific products or items for the Work and provide the requested information for each supplier/manufacturer. 5.1.1 Submit List of Suppliers using the attached template. Group 6.1: Social Sustainability Instructions: 6.1.1 Fair Wage: Confirm that the Tenderer has reviewed the Fair Wage conditions described in Part A – Introduction of this ITT and will comply with the applicable requirements set out in the Form of Agreement 6.1.2 Describe the Tenderer’s hiring practices for industry/sector/trade apprenticeships and/or skills training. 6.1.3 Describe any specific methods the Tenderer uses to improve diversity in hiring and create opportunities for under-represented populations in trades (such as Indigenous persons, women, youth, minorities, persons with disabilities). Group 6.2: Environmental Sustainability Instructions: 6.2.1 Toxicity (water/air): List any products or services contemplated in the Tender that are toxic or hazardous to the environment or humans and complete the following table in relation thereto. Include the following information for each product or service listed: -Description of Toxin/Hazard -Substantiation for Use -Mitigation strategy to reduce or dispose the Toxin/Hazard 6.2.2 Waste: Identify the Tenderer’s standard practices for reduction, diversion or disposal of obsolete or expired materials, products or equipment. Group 7.1: Insurance Instructions: 7.1.1 Acknowledge that a Certificate of Insurance will be completed if awarded the contract. 7.1.2 Complete and attach a Certificate of Existing Insurance and an Undertaking of Insurance. Group 8.1: Supplier Code of Conduct Instructions: All Tenderers must declare compliance with the Supplier Code of Conduct. 8.1.1 The City of Vancouver expects each supplier of goods and services to the City to comply with the City’s Supplier Code of Conduct (SCC) referenced in Part A - Introduction. The SCC defines minimum labour and environmental standards for City suppliers and their subcontractors. Suppliers are expected to comply with the aforementioned standards upon submitting a tender, proposal, application, expression of interest or quotation to the City, or have a plan in place to comply within a specific period of time. The City reserves the right to determine an appropriate timeframe in which suppliers must come into compliance with these standards. The Tenderer declares that it has reviewed the SCC and to the best of its knowledge, the Tenderer and its proposed subcontractors have not been and are not currently in violation of the SCC or convicted of an offence under national and other applicable laws referred to in the SCC, other than as disclosed below. 8.1.2 Violations of Supplier Code of Conduct, if any. Outline all violations/convictions occurred in the past three years and provide the following information for each violation/conviction: - Section of SCC/title of law - Date of violation/conviction - Description of violation/conviction - Regulatory/adjudication body and document file number - Corrective action plan Group 8.2: Conflict of Interest Declaration Instructions: All Tenderers must disclose any actual or potential Conflict of Interest. 8.2.1 The Tenderer confirms and warrants that there is no officer, director, shareholder, partner, employee or contractor of the Tenderer or of any of its proposed subcontractors, or any other person related to the Tenderer’s or any proposed subcontractor’s organization (a “person having an interest”) or any spouse, business associate, friend or relative of a person having an interest who is: (a)an official or employee of the City; or (b)related to or has any business or family relationship with an elected official or employee of the City, in each case, such that there could be any conflict of interest or any appearance of conflict of interest in the evaluation or consideration of the Tender by the City, and, in each case, except as set out, in all material detail in the section titled “Disclosure” below. 8.2.2 The Tenderer confirms and warrants that there is no person having an interest (as defined above) who is a former official, former employee or former contractor of the City and who has non-public information relevant to the ITT obtained during his or her employment or engagement by the City, except as set out, in all material detail, in the section titled “Disclosure” below. 8.2.3 The Tenderer confirms and warrants that neither the Tenderer nor any of its proposed subcontractors is currently engaged in supplying (or is proposing to supply) goods or services to a third party such that entering into an agreement with the City in relation to the subject matter of the ITT would create a conflict of interest or the appearance of a conflict of interest between the Tenderer’s duties to the City and the Tenderer’s or its subcontractors’ duties to such third party, except as set out, in all material detail, in the section titled “Disclosure” below. 8.2.4 The Tenderer confirms and warrants that: (a)the Tenderer is not competing within the ITT with any entity with which it is legally or financially associated or affiliated; and (b)the Tenderer is not cooperating in any manner in relation to the ITT with any other tenderer responding to the ITT, in each case, except as set out, in all material detail, in the section titled “Disclosure” below. 8.2.5 The Tenderer confirms and warrants that: (a)neither it nor any officer, director, shareholder, partner, employee or agent of the Tenderer or any of its proposed subcontractors is registered as a lobbyist under any lobbyist legislation in any jurisdiction in Canada or in the United States of America; and (b)neither it nor any officer, director, shareholder, partner, employee or agent of the Tenderer or any of its proposed subcontractors has engaged in any form of political or other lobbying whatsoever with respect to the Tender or sought, other than through the submission of the Tender, to influence the outcome of the ITT, in each case, except as set out, in all material detail, in the section titled “Disclosure” below. 8.2.6 Disclosure: Provide any disclosures related to the conflict of interest declarations above.
