Anticipated Annual Award Amount: Up to $30,000 per year, up to 3 years. The agency funded under this survey will be responsible for providing consultation to the Wisconsin Department of Health Services (DHS) and other partners on how to foster positive social connections for maternal and child health (MCH) populations within our existing public health infrastructure and programs. The Wisconsin Title V program defines social connections as follows: Social Connection refers to the ways people are physically, emotionally, and socially connected to others. When people have a sense of belonging (they feel valued and included) their health and well-being are positively influenced. Grant funding is expressly dependent upon the availability of funds to the Wisconsin DHS from the Health Resources and Services Administration, Title V Maternal and Child Health Grant. Applicants will be awarded based on available funding, capacity, and ability to meet application criteria. This project begins January 1, 2023 and ends December 31, 2025. Each contract will have a duration of one calendar year with the possibility of annual renewal for the duration of the project period, contingent on funding availability and performance. Should additional funding become available, award amounts may be altered.
