County of Los Angeles Department of Public Works is seeking proposals for Forklift Operator Safety Training and Certification for Heavy Equipment with Forklift Attachments (Refresher) - 4 hours. After reviewing the information contained in the attachment, please submit all items requested within the Request for Proposal (RFP) by email to mtran@dpw.lacounty.gov or by fax to (626) 282-8312 no later than Thursday, September 13, 2018.

All bid proposals must be on company

letterhead. Late and/or incomplete proposals may not be accepted.
Vendors must be a registered County Vendor. You can register at http://camisvr.co.la.ca.us/LACOBids/

IMPORTANT NOTE: If you are unable to participate in this solicitation, please submit a �No Bid� response to mtran@dpw.lacounty.gov.

Proposals will have examined in accordance with the goals and objectives outlined in this RFP and will be evaluated to determine if training curriculum, manuals, approach, experience and qualifications demonstrate that the Proposer is likely to meet or exceed expectations.
