The County of Santa Clara (hereafter, “County”), dba Valley Health Plan (hereafter, “VHP”) is requesting proposals from qualified vendors to provide a service of
Health Risk Assessment (HRA) and web-based health management tools that are required for accreditation for both Accreditation Association for Ambulatory Health Care (AAAHC) and National Committee for Quality Assurance (NCQA).  
Through this Request for Proposal (RFP), VHP intends to enter into an agreement with the most qualified Proposer that best meets the requirements of VHP.
 
This RFP will result in a single non-exclusive contract award. If an award is made, it is the County’s intent to award a contract to a single non-exclusive supplier, with a term of two (2) years with an option to renew for three (3) additional years, unless terminated earlier or otherwise amended.  
 

THIS RFP SOLICITATION REQUIRES ALL PROPOSALS TO BE SUBMITTED ELECTRONICALLY THROUGH PERISCOPE HOLDINGS.  NO PAPER, EMAIL OR FAX SUBMISSIONS WILL BE ACCEPTED AND MAY BE DEEMED NONRESPONSIVE.
