Telehealth Intervention Programs for Seniors

REQUEST FOR PROPOSAL : Telehealth Intervention Programs for Seniors The Westchester County Department of Senior Programs and Services (“Department”) invites proposals from qualified proposers to provide Telehealth Intervention Programs for Seniors, in accordance with the terms and conditions of the Coronavirus Local Fiscal Recovery Fund (“CLFRF”) of the American Rescue Plan (“ARP”) Act of 2021. Services will be provided to elderly persons aged sixty (60) years or older residing in Westchester County. These services will enable older persons to self-manage chronic conditions, avoid unplanned and emergency medical visits, and reduce medical expenses for the participants.
