Off-Site Medical Services for Employees and Dependents

City of Lubbock is soliciting Request for Proposal (RFP) from interested parties to provide off-site medical services to employees and dependents enrolled in the City of Lubbock Health Care Plan as described in the Scope of Services attached hereto as Exhibit "A".  The City is not interested in an on-site clinic; therefore, any proposals for such will be deemed as non-responsive
 

Due Date and Required Documents  
One reproducible original and four (4) bound copies of the response, including all required forms and applicable supporting documentation are required.
 
Responses must be submitted not later than
3:00 p.m. Central Standard Time on Tuesday, September 18, 2018 .  The responses must be bound and sealed when submitted.  The response material must be addressed and delivered to:
