Can you please help me by sending a link or sending me the needed documents? The PQS form (AE12a) can be found on the Department’s webpage at https://www.dms.myflorida.com/business_operations/re
al_estate_development_and_management/building_con struction/forms_and_documents. 2 In line 1 of the PQS form (AE12a) titled ‘PROJECT NUMBER’, should we list the project number as N/A as shown on the advertisement? Or should we include the RFQ project number (RFQ-REDM17/18- 010) instead? Yes, the project number may be listed as “N/A”. 3 Since this is an architectural minor services contract, we will not have a proposed team of subcontractors/consultants (i.e.- The Department will only review the qualifications of the prime firm. Is there a specific person or selection committee member (i.e. Attention: Ryan Meikenhous or someone else)? The Letter of Interest may be addressed to the Committee or Ryan Meikenhous. 5 Is there a page number limit to the submittal? There is no page number limit to the submittal. 6 Can we add multiple engineering firms for the same discipline to allow for reciprocity. If so, how do we show that on the PQS form? The Department will only review the qualifications of the prime firm. 7 Are we to address the interview portion of the grading criteria in the proposal phase? For example: References, Understanding of Program and Methodology? No. 8 Is there a page limit? There is no page number limit to the submittal. 9 Are we allowed to put the PQS or sf330 forms in our own format as long as the information remains the same order. No, please use the templates as available online. 10 Do you need license information on only the prime firm or all firms? The Department will only review the qualifications of the prime firm. 11 Do you need license information on architects and engineers? Or just firm? The Department will only review the qualifications of the prime firm however, you may submit all licenses. 12 Is the PQS firm only to be filled out by the prime for sections 4, 5 & 7? Yes. The Department will only review the qualifications of the prime firm. Any PDF format is acceptable as long as all sections are included. 14 Please confirm that electronic PDF version is a combined PDF of all sections not individual PDF’s of each section. The electronic PDF version can be one combined file or each section can be its own PDF. 15 Location of proposed project (column J): If no projects have been identified what distance should we specify? The prime firm does not specify a distance. The prime firm’s office in relation to the project (in this case a region of projects) will determine the score for Column J. 16 Will Column M be measured by Section’s E and F in the SF330? No, the individual committee members will determine the score of Column M after reviewing the firm’s submittal. 17 What types of projects or tasks for Professional Services, (or examples of tasks) will be assigned to the Architectural firms? Tasks can include but are not limited to studies, renovations, as-builts, ADA related items, etc. 18 The estimated construction costs, planning in what field? The construction costs can be any amount up to $2,000,000.00. AE-Continuing Contracts - Northwest 1 STATE OF FLORIDA, DEPARTMENT OF MANAGEMENT SERVICES REAL ESTATE DEVELOPMENT AND MANAGEMENT PUBLIC ANNOUNCEMENT FOR PROFESSIONAL SERVICES FOR CONTINUING CONTRACTS - ARCHITECTURAL SERVICES NORTHWEST FLORIDA REGION June 5, 2018 RFQ Number: RFQ-REDM17/18-010 Project Number: NA Project Name: Architectural Services Continuing Contracts Project Location: Northwest Florida Region The State of Florida, Department of Management Services (DMS), Division of Real Estate Development and Management, requests qualifications from Architectural firms to provide Professional Services primarily in the Northwest Florida Region. These services may be used for projects anywhere in the State of Florida, however, it is anticipated that the vendors selected during this process will be activated for projects in the Northwest part of the State. The Northwest Region is defined as all counties west of and including Taylor and Madison County. A minimum of three (3) firms for the Northwest Florida Region may be selected under this Continuing Contract and will be responsible for assigned projects having estimated construction costs, planning or study fees not exceeding the threshold amounts of $2,000,000.00 (construction) and $200,000.00 (planning or study fees) respectively, provided for in Section 287.055, Florida Statutes. This contract will be for an initial period of two (2) years with an option to renew for an additional two-year period. QUALIFICATIONS SELECTION CRITERIA: Firms must be properly licensed in the State of Florida at the time of submittal. Firms are advised that plans and specifications for Architect Engineer projects may be reused. Selections will be made in accordance with Chapter 287.055, Florida Statues. INTERVIEWS: The Selection Committee will conduct interviews with and may require public presentation by no fewer than three (3) firms regarding their qualifications, approach to the project, and ability to furnish the required services. Selection of finalists for interview will be made on the basis of professional services qualifications including related architectural experience and ability of professional personnel, location, billable staff, recent, current and projected workloads of the firms, Florida licensed staff, and volume of DMS and other State Agency work. The list of firms selected for interview by the Selection Committee will be posted on the DMS website. Each invitee will be notified by AE-Continuing Contracts - Northwest 2 fax/email of the date and time of its interview/presentation. All interviews will be held at the following address: Department of Management Services, Division of Real Estate Development and Management, 4050 Esplanade Way, Tallahassee, Florida 32399. INSTRUCTIONS: Firms interested in being considered for this project must submit five (5) printed copies and one scanned copy of the entire response in Adobe (.pdf) on a thumb drive of their submittal with a table of contents and tabbed sections in the following order: 1. A Letter of interest detailing the firm's qualifications, related experience, the firm's abilities to do the work, and to meet the above referenced selection criteria. 2. Professional Qualifications Supplement (PQS). Specify number of billable staff within the office address listed in Item 2, whether or not you have current or previous DMS or Agency State Work. (You must use “DMS Form AE12a-Rev 7/12” which may be obtained from the DMS website under Forms and Documents). 3. A copy of the firm's current Florida Department of Business and Professional Regulation License. 4. For Corporations only: If the firm offering services is a corporation, it must be properly registered with the Florida Department of State to practice their profession in Florida and must provide a copy of the firm’s current Florida Corporate Registration. 5. Completed Federal GSA Standard Form 330 (Rev. 8/2016). (This Form may be downloaded at http://www.gsa.gov/). (These documents may be obtained from the DMS website under Forms and Documents). SELECTION SCHEDULE: The table below contains the Timeline of Events for this advertisement. Firms shall become familiar with the Timeline of Events. The dates and times within the Timeline of Events may be subject to change. It is the firm’s responsibility to check for any changes. All changes to the Timeline of Events will be made through an addendum to the advertisement on the Vendor Bid System (VBS) website. Firms are responsible for submitting all required documentation by the dates and times (Eastern Time) specified below. Respondents to this solicitation or persons acting on their behalf may not contact, between the release of the solicitation and the end of the 72-hour protest period following the agency posting the notice of intended award, excluding Saturdays, Sundays and state holidays, any employee or officer of the executive or legislative branch concerning any aspect of this solicitation, except in writing to the procurement officer or as provided in this solicitation.
