The Napa County Health and Human Services Agency desires to solicit qualified proposals to provide Youth Partner services, one (1) youth partner for youth in foster care or formerly in foster care. The youth partner will provide ongoing support to ILP eligible youth ages 14 1⁄2 to 21. This support will include peer support during Child Family Team Meetings, court hearings and family visitation as requested. In addition, the youth partner will be available to support youth in meeting the services objectives and goals outlined in the transitional living plan to accomplish education, training and employment goals. The Youth Partner will also provide consultation to policy and procedure development and program changes as a subject matter expert with “lived experience” in the Child Welfare system.The Youth Partner shall work in partnership with Napa County HHSA- Child Welfare Services and will contribute the voice of a youth with lived experience in the foster care system. The Youth Partner will participate in Child and Family Team Meetings, provide mobile response as needed for Family Urgent Response System (FURS) calls and will provide input on the development of Napa County HHSA - Child Welfare Policies and Procedures. The Youth Partner will work to connect transitional age youth, to the resources and services they need in order to achieve academic, employment, and self-sufficiency success through outreach, resource navigation, and follow-up while developing a peer supportive relationship.
