The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide Dermatopathology Consultation Services for the tissue samples received from, Veteran Affairs Medical Center (VAMCSLC), 500 Foothill Drive, Salt Lake City, UT 84148 The VAMCSLC Pathology & Laboratory Department will cut and stain\dermatopathology slides and send them to the contractor for dermatopathology consultative services on cases requiring a second opinion from experts specializing in Dermatopathology (tests showing a possible pathology such as cancer). The contractor shall guarantee same day or overnight transportation of stained slides (via courier service, UPS, or FedEx). The contractor shall provide the VAMCSLC Pathology & Laboratory Department with the necessary shipping supplies (i.e., slide containers, bubble wrap, transport container, UPS/FedEx envelopes and prepaid overnight labels, etc.) for shipping the

original consult case, additional slides, and any other correspondence at no additional charge to the government. The contractor shall provide all necessary supplies normally provided to its commercial customers, not limited to the following: Requisition forms Specimen containers and pour off tubes Dry ice and appropriate container Special instructions Current list of tests with reference ranges, specimen requirements, and test descriptions Specimen carriers Special media or special tubes for samples All forms necessary for specimens All supplies for special tests The NAICS code is 621511 Medical Laboratories and Small Business Size Standard is $22 Million The information submitted shall contain the company s business size status.
