Description: Pursuant to the State Purchasing Act (Official Code of Georgia Annotated §§50-5-50 et seq.), this electronic Request for Proposals (eRFP) is being issued to establish a contract with a qualified supplier who will provide one (1) supplier who will provide Laserfiche - Document Management Services including development, software, support and training to the Georgia Department of Community Health (DCH) (hereinafter, "the State Entity") as further described in this eRFP. DCH will expect the selected supplier to submit a binding Statement of Work (SOW) for each new project which will propose time and materials and not to exceed for the project with the exception of a change order. If approved by DCH the supplier will be expected to work within the authorized budget and deliver the project within the established time and requirements

parameters. DCH first obtained and started using Laserfiche in a single work unit through a reseller contract in 2009. This initial site utilized Laserfiche's core content management capabilities and Laserfiche Quick Fields image processing and integrating with external databases. DCH is using Laserfiche as an integration platform to enhance automation and improve business processes (See Exhibit 1 below). There are hundreds of internal and web-facing forms and workflows in production which provide business critical transactions. Workflows and forms utilize a combination of native Laserfiche functionality and custom coding and will require ongoing vendor support from expert developer resources. Since 2009 the agency has substantially expanded the scope of those services as the business needs have grown such that Laserfiche has become an enterprise-wide content and workflow/business process management system. Currently, DCH's Enterprise Content Management (ECM) contract includes 936 user licenses across several critical business units.
