The intent of this request for proposal (RFP) is to develop and implement a customized community-based provider support service model for Medicaid Members with Intellectual Disabilities and co-occurring Complex Behavioral Health Needs in four areas (See Attachment G, Geographical Area Map) across Iowa. This service model will address identified gaps in staff training and supports impacting services and supports for Medicaid members utilizing home and community-based services (HCBS) Intellectual Disability Waiver or Habilitation Services. Each of the four Geographical Areas will provide services within a defined area of the state to service providers through specialized consultation, technical assistance, Crisis intervention, Crisis Stabilization and Care Coordination to ensure Iowans with Complex Behavioral Health Needs can remain living in integrated settings of their choice. The service array must include both in-person and virtual services and supports provided in the member’s home. The Geographical Area may be modified by the Contractor with approval from the
