2 Statement of Work Northern Arizona VA Healthcare System Satellite TV Installation, Subscription, & Service Requirement June 16, 2022 SCOPE: The Northern Arizona VA Healthcare System (NAVAHCS) requires a commercial grade Satellite TV solution in its patient care rooms/areas, with a minimal channel line-up, including installation, subscription, and maintenance service. The Contractor shall provide all necessary labor, supervision, material, and equipment to install Satellite TV at the facility. This requirement is applicable to four (4) buildings with a total of 134 televisions, locations provided below plus an additional 10 to provide an error of margin or add-ons totaling to 144. The campus is located at the Northern Arizona VA Healthcare System (NAVAHCS) 500 Highway 89N, Prescott, AZ 86313 and many buildings are on the Arizona Historical Registry. No satellite devices, antennas, or dish(es) shall penetrate roof structures or sides of buildings, and other restrictions/considerations
may apply. NAVAHCS suggests use of a weight appropriate sled if needed. Locations of satellite dish(es), antenna(s), headend equipment, converters etc. must be pre-approved by COR or VA designee prior to install. The placement of satellite(s) and conduit/cable must not be visible at ground level. NAVAHCS uses an interactive patient care TV system at its 118 bedsides/patient rooms requiring a non ProIdiom signal. These TVs are Flytech Model K948. Any equipment installed must be able to deliver television service that does not use a transitional box to decode signal. GENERAL REQUIREMENTS: NAVAHCS buildings are not connected by coax however are all connected with fiber. NAVAHCS envisions a solution where the headend has the capability for IPTV using a media converter. Contractor shall provide all Labor, Parts, Materials, Antennas, Satellites, Cabling, Equipment, Hardware, Remote Controllers, Software, Programming and Technical Support to ensure Television Subscription Service connectivity to all indicated televisions (TVs) within NAVAHCS. Contractor shall provide Tower Racks if needed or the equivalent to store and house the equipment used to provide cable Television Subscription Service. NAVAHCS will provide space in an equipment room and electric outlets as necessary to power front end equipment. Other concessions shall be negotiated between the Contractor and NAVAHCS on an as needed basis. Contractor shall provide their own internet connection during the installation process of all software and channel programming. NAVAHCS will not furnish parts or test equipment for the duration of the contract. It is the responsibility of the Contractor to provide the essential equipment and supplies to complete all work relevant to this contract. Bidder shall submit a Basic 48-60 customizable channel line-up with bid submission to include main local channels (FOX, NBC, CBS, ABC). System must be able to operate without having cable boxes at each TV. SERVICE: Contractor shall be responsible for all repairs/maintenance to all installed equipment. Contractor shall be able to provide a dedicated sales representative/account manager, and a customer service contact phone number. Customer service contact phone number shall be answered by a live person, twenty-four (24) hours a day, and seven (7) days a week. Contractor shall respond to all request for service calls within (24) hours of the request. WARRANTIES: Headend equipment to the satellite, including the satellite, and over the air antenna (OTA) must have at least a 5-year warranty. INSTALLATION: The Contractor shall check-in and check-out with the Police Service located in Building 28. The Contractor must also be accompanied by the VA designee at all times during installation. All installation services shall be performed between the hours of 7:30am 4pm excluding Federal holidays. All debris shall be removed from the premises and properly disposed of by the Contractor. FEDERAL HOLIDAYS: The following national holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Juneteenth 19 June Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December If a holiday falls on Sunday, the following Monday shall be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States. SAFETY: Contractor shall always be required to wear Personal Protective Equipment (PPE) during the performance period. If needed, and depending on the scope of work, the Contractor must attain the correct type of permit required by VHA Safety Office. Work cannot commence until the appropriate work permit has been signed off by all parties. The following permits shall apply depending on the scope of work: Hot Work Permit Above the Ceiling and Penetration Permit Energized Electrical Permit Utility and Fire Alarm System Permit INVOICE AND PAYMENT: Payment to be made monthly in arrears. The Vendor shall submit original invoices in proper electronic format to the Financial Services Center (FSC). Proper electronic format means an automated system transmitting information electronically according to the accepted electronic data transmission methods below: VA s Electronic Invoice Presentment and Payment System The FSC uses a third-party contractor, OB10, to transition vendors from paper to electronic invoice submission. Please see OB10 contact information below to begin submitting electronic invoices, free of charge. A system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) chartered by the American National Standards Institute (ANSI).
